Résumé

Patiente âgée de 50 ans, sans antécédents, a consulté pour une masse cervicale antérieure évoluant depuis 10 ans compliquée d’une dysphonie depuis 6 mois. L’examen clinique a retrouvé une masse cervicale antérieure et latérale, dure, indolore et mobile à la déglutition. La nasofibroscopie a objectivé une paralysie de la corde vocale gauche avec bombement réduisant la filière de l’étage sus-glottique. Un scanner cervico-thoracique a été réalisé montrant un processus tumoral thyroïdien envahissant le larynx. Une biopsie thyroïdienne a été réalisée sous anesthésie locale montrant un carcinome papillaire thyroïdien à différenciation vésiculaire exclusive. Le traitement a consisté en une résection en bloc de la thyroïde, du larynx et un évidement ganglionnaire central et latéral, suivi d’une ira-thérapie de 100 mCi.
Le traitement chirurgical des cancers thyroïdiens avec envahissement laryngo-trachéal a pour but de retirer l’ensemble du tissu pathologique tout en conservant les fonctions respiratoires, digestives et phonatoire. Les exérèses élargies exposent à une morbidité importante. Cependant elles représentent le seul moyen d’éviter une compression respiratoire permettant d’augmenter la durée de la survie et elles sont dans certains cas curatifs.

Papillary thyroid carcinoma with laryngeal invasion

A woman aged 50 years consulted for an anterior cervical mass evolving for 10 years complicated by dysphonia for 6 months.Physical examination found an anterior and lateral neck mass, hard, painless and mobile with swallowing. Physical examination found an anterior and lateral neck mass, hard, painless and mobile swallowing. The nasofibroscopy objectified paralysis of the left vocal cord with bulging reducing the supraglottic space. A cervico-thoracic CT scan was performed showing a thyroid tumor process extended to the larynx. Thyroid biopsy under local anesthesia finding a papillary thyroid carcinoma with exclusive follicular differentiation. The treatment consisted of a complete resection of the thyroid, larynx, central and lateral lymph node dissection. Followed by 100 mCi of IRA-therapy.
Surgical treatment of thyroid cancer with laryngotracheal invading has for objective to remove the entire pathological tissue with preserving respiratory, digestive and vocal functions. Extended resections exposed to significant morbidity. However, they are the only way to avoid respiratory compression to increase the survival time and are curative in some cases.
