Introduction : La Sténose de l’Artère Rénale du Transplant (SART) est une complication vasculaire fréquente en transplantation rénale pouvant s’accompagner d’une hypertension artérielle et/ou d’une dysfonction de l’allogreffe. Notre travail se propose d’étudier la prévalence de la sténose et d’évaluer les résultats de l’angioplastie transluminale per cutanée comme traitement de premier choix.
Patients et méthodes : Nous avons étudié rétrospectivement les paramètres cliniques, biologiques et thérapeutiques des patients transplantés rénaux du service. Deux groupes de patients ont été identifiés ceux présentant ou non une sténose de l’artère rénale du transplant.
Résultats : Quatorze patients (11,8%) ont développé une sténose de l’artère rénale du transplant, 11 hommes et 3 femmes dont l’âge moyen est de 36,0 ± 9,5 ans. Cette sténose était apparue 91 ± 72 jours (10-248 j) de la transplantation, elle était au niveau du site anastomotique chez tous les patients. Une Angioplastie Transluminale per cutanée (ATL) avec mise en place d’un stent était réalisée chez 6 patients, chez qui la sténose avait compromis la fonction du greffon ou l’hypertension artérielle était difficilement contrôlable. Une seule patiente a perdu son greffon rénal après angioplastie. La pression artérielle était contrôlée chez 3 patients par une seule drogue, et chez les 3 autres patients, la pression artérielle s’était normalisée sans aucun traitement antihypertenseur. Chez les 8 autres patients chez qui la sténose n’était pas significative ; on a opté pour une surveillance échographique régulière et optimisation du traitement antihypertenseur.
Conclusion : La sténose serrée de l’artère rénale du transplant peut être traitée initialement avec succès par une angioplastie transluminale per cutanée.
Introduction: Transplant Renal Artery Stenosis (TRAS) is a common vascular complication in renal transplantation, which may be accompanied by high blood pressure and/or allograft dysfunction. Our work aims to study the prevalence of this stenosis and to evaluate the results of percutaneous transluminal angioplasty as the first-choice treatment.
Patients and methods: We retrospectively studied the clinical, biological and therapeutic parameters of renal transplant patients in our center. Two groups of patients were identified, those with or without transplant renal artery stenosis.
Results: Fourteen patients (11.8%) developed transplant renal artery stenosis (TRAS), 11 men and 3 women with an average age of 36.0 ± 9.5 years. This stenosis appeared 91 ± 72 days (10-248 days) post-transplant; it was at the anastomotic site in all patients. A percutaneous transluminal angioplasty (PTA) with stent placement was performed in 6 patients, in whom the stenosis had compromised graft function or hypertension was difficult to control. Only one patient lost her kidney transplant after angioplasty. Blood pressure was controlled in 3 patients by a single antihypertensive drug, and in the other 3 patients, blood pressure normalized without any antihypertensive treatment. In the 8 other patients in whom the stenosis was not significant; we opted for regular ultrasound monitoring and optimization of antihypertensive therapy.
Conclusion: The significant transplant artery stenosis can be successfully treated initially by percutaneous transluminal angioplasty.
