Introduction : Une perforation de la membrane tympanique est une perte de substance créant une communication anormale entre l’oreille moyenne et le conduit auditif externe. La tympanoplastie est une intervention réparatrice de cette perforation, elle a un objectif ultime de reconstruire la membrane tympanique et de restaurer ou préserver la fonction auditive.
Le but de ce travail était d’analyser les résultats anatomiques et fonctionnels des tympanoplasties par technique underlay avec une revue de littérature.
Matériel et méthodes : Il s’agit d’une étude rétrospective mené sur 202 patients sur une période de 2 ans (janvier 2014-janvier 2016) au niveau du service d’ORL et chirurgie cervico-faciale hôpital 20 août, CHU Ibn Rochd de Casablanca.
Résultats : L’âge moyen des patients était de 41,88 ans avec une prédominance féminine (sex-ratio de 0,29). Vingt-quatre patients opérés (11,88%) présentaient des cas de reprise chirurgicale. Après un recul moyen de 12 mois, 188 cas (93,07%) présentaient un tympan totalement fermé, 4 cas (1,98%) avec une rétraction et 10 cas (4,95%) avec une perforation résiduelle. Le gain auditif moyen était de 8,64 dB. Selon le test de Wilcoxon, il y avait une amélioration significative de l’audition (p < 0,05).
Conclusion : La tympanoplastie représente une technique efficace et saine pour améliorer la qualité de vie des patients souffrant d’une perforation tympanique.
Introduction: Perforation of the tympanic membrane is a defect creating an abnormal communication between the middle ear and the external auditory meatus. It has an ultimate goal of reconstructing the tympanic membrane and to restore or preserve the hearing function. The aim of this study was to analyze the anatomical and functional results of tympanoplasty by technical underlay with a literature review.
Materials and methods: This is a retrospective study conducted on 202 patients over a period of 2 years (January 2013-January 2015) at the ENT department and Neck Surgery Hospital 20 août, UTH Ibn Rochd of Casablanca.
Results: The mean age of patients was 41.88 years with a female predominance (sex ratio 0.29). After a mean of 12 months, 188 cases (93.07%) had completely closed eardrum, 4 cases (1.98%) with a retraction and 10 cases (4.95%) with residual perforation. The average hearing gain was 8.64 dB. According to the Wilcoxon test, there was a significant improvement of hearing (p < 0.05).
Conclusion: The tympanoplasty is an effective and safe technique for improving the quality of life of Patients affected a tympanic perforation.
