Introduction : L’adénocarcinome gastrique à cellules indépendantes (ADCI) est une entité de cancer gastrique particulière. L’histologie correspond à un adénocarcinome avec une composante à cellules indépendantes majoritaire (>50 %). L’ADCI apparaît plus agressif, moins sensible à la chimiothérapie, et avec un pouvoir métastatique plus important que l’adénocarcinome gastrique. Ce travail a pour objectif de discuter des particularités histologiques, pronostiques et thérapeutiques de ce cancer.
Matériels et méthodes : Dans ce travail rétrospectif, nous avons analysé 57 observations de patients présentant une tumeur gastrique à cellules indépendantes, traités au service d’oncologie médicale au CHU Hassan II de Fès entre janvier 2012 et décembre 2015.
Résultats : L’âge médian des patients au moment du diagnostic était de 50 ans. Quarante-deux pour cent des patients étaient des femmes. La maladie a été révélée par des épigastralgies dans 47% des cas. Le délai médian du diagnostic était de 5 mois. La tumeur primitive était de siège antral dans 41% des cas. Soixante-quatre pour cent des patients avait un stade IV de leur maladie au moment de diagnostic, elle était associée dans la plupart des cas à une carcinose péritonéale généralisée (52%), et à des métastases hépatiques (20%). Soixante-dix pour cent de ces patients ont reçu une chimiothérapie palliative, et 10% ont reçu des soins de supports. Quatre-vingts pour cent ont reçu le protocole ECX en première ligne. Trente-six pour cent avait une tumeur localisée (stade II, III), ils ont eu une chimiothérapie péri-opératoire dans 71% des cas, et 29% ont été opérés d’emblée. La durée médiane de contrôle de la maladie chez ces patients était de 6 mois. La médiane de Survie Sans Récidive (SSR) était de 12 mois et la médiane de Survie Globale était (SG) est de 26 mois.
Conclusion : L’incidence de l’ADCI augmente ces dernières années. Leur pronostic est équivalent à celui des adénocarcinomes gastriques. En revanche, l’ADCI est une tumeur avec un potentiel d’agressivité supérieur à celui des adénocarcinomes gastriques. Le traitement actuellement recommandé reste la chimiothérapie péri-opératoire associée à une gastrectomie totale.
Introduction: Independent cell gastric adenocarcinoma (ADCI) was a special gastric cancer entity. The histology corresponds to an adenocarcinoma with a majority independent cell component (> 50%). ADCI appears more aggressive, less sensitive to chemotherapy, and with greater metastatic power than gastric adenocarcinoma. The purpose of this work was to discuss the histological, prognostic and therapeutic features of this cancer.
Materials and methods: In this retrospective study, we analyzed 57 cases of patients with a gastric tumor with independent cells, treated in the department of medical oncology at the CHU Hassan II of Fez between January 2012 and December 2015.
Results: The median age of the patients was 50 years old. 42% of the patients were women. Gastralgia was the revealing symptom in 47% of the cases. The median time to diagnosis was 5 months. The predominant localization was antral in 41% of cases. Sixty four percent of the patients had a stage IV disease at the time of diagnosis. The most common secondary localization was peritoneum (52%) and liver (20%). Seventy percent of these patients received palliative chemotherapy and 10% received supportive care. Eighty percent of the patients received first line therapy based on ECX protocol. Thirty six percent of the patients had a localized tumor (stage II, III), 71% of whom received perioperative chemotherapy. The median duration of disease control in these patients was 6 months. The median recurrence free survival (RFS) was 12 months and the median overall survival was (OS) was 26 months.
Conclusion: The incidence of ADCI has increased in recent years. Their prognosis is equivalent to that of gastric adenocarcinomas. In contrast, ADCI is a lesion with an aggressive potential greater than that of gastric adenocarcinomas. The currently recommended treatment is perioperative chemotherapy associated with total gastrectomy.
