Objectifs : Evaluer le profil clinique et analyser les facteurs pronostiques du cancer du rein.
Matériel et méthodes : 130 cas de cancers du rein ont été colligés dans le service d’urologie «B» du CHU Ibn Sina de Rabat durant une période de 13 ans (janvier 1989 - décembre 2002). Pour chaque patient, nous avons étudié les circonstances de découverte, les résultats des examens biologiques et radiologiques, le traitement appliqué, le profil anatomo-pathologique (stade et grade), l’évolution ainsi que les facteurs pronostiques. La plupart de nos patients a été revue à la consultation avec un recul suffisant.
Résultats : Les patients se répartissent en 64 femmes (49 %) et en 66 hommes (51 %), d’un âge moyen de 55 ans (18-85 ans). La symptomatologie clinique était polymorphe, dominée par des une hématurie (40 %), douleurs lombaires (25 %), une masse lombaire (14 %) et l’altération de l’état général (6 %). Le diagnostic était basé sur l’échographie chez 91 patients et l’uroscanner chez 126 patients. La taille tumorale moyenne est de 10 cm (3 -20 cm). Le traitement était chirurgical chez 113 patients (87 %) par voie antérieure sous costale dans 75 % des cas. La néphrectomie totale élargie a été pratiquée chez 108 patients (83 %) et une néphrectomie partielle a été réalisée chez 5 patients (4 %). Le carcinome à cellules claires était présent dans 84 % des cas. Les ganglions prélevés sont envahis dans 26 % des cas. Le suivi moyen était de 35 mois (7-98 mois). La survie sans récidive était de 58 % à un an (66 patients) et de 47 % à 5 ans (53 patients) tous stades confondus. Une récidive tumorale au niveau de la loge a été retrouvée chez 4 patients (3,5 %). Les métastases asynchrones sont survenues au cours de l’évolution chez 9 patients (8 %) après un délai moyen de 21 mois (12 - 48 mois).
Conclusion : II apparaît clairement dans notre série que l’amélioration du pronostic devrait passer obligatoirement par un diagnostic précoce. Le développement de l’imagerie (échographie, scanner) a permis de réduire la fréquence des tumeurs diagnostiquées tardivement.
Purpose : To evaluate the clinic profile and analyze the prognosis factors of kidney cancer.
Materials and methods : The authors report a series of 130 cases of renal cancer observed over a 13 year period (January 1989 - December 2002) in urology department “B” of Ibn Sina University hospital center of Rabat. For every patient, we studied the clinical presentation and the results of biological and radiological exams, the applicated treatment, and the profile of histology, the evaluation as well as prognostic factors. Most of our patients were reviewed in the consultation.
Results : Patients consisted of 64 women (49%) and 66 men (51%), with a mean age of 55 years (range : 18-85 years). The clinical features were polymorphic dominated by haematuria (40%), loin pain (25%), a lumbar mass (14%), alteration of the general state (6%). The diagnosis was established by ultrasonography in 91 patients and CT-scan in 126 patients. The mean tumour diameter was 10 cm (3-20 cm) and two cases presented bilateral tumours. Time to diagnosis ranged from 1 month to 7 years. Staging reflected the advanced stage of the cancer. Treatment was surgical for 113 patients (87%). A lumbar incision was performed in 25 of the cases. Radical nephrectomy was performed in 83 %. And partial nephrectomy was performed in 4% of patients. Histological examination of the specimen showed renal cell carcinoma in 84 % of the cases. The lymph nodes removed were invaded in 26 % of the cases. The mean follow-up was 35 months (4 to 96 months) normal at one year for 66 patients (58%) and at 5 years for 53 patients (47%). Tumour recurrence in the renal compartment was observed in 4 patients (3,5%). Asynchronous metastases occurred in 9 patients (8%) after 21 months (range :12-48 months).
Conclusion : It is clearly apparent in our series, that the improvement of prognosis, should obligatory go through an earlier diagnosis. The development of the imagery (ultrasound, computed tomography) has permitted the decrease incidence of advanced disease and metastatic renal cell cancer.
