Résumé

Le pied de Charcot est une ostéoarthropathie nerveuse, survenant essentiellement chez le sujet diabétique et qui pose le problème de diagnostic positif et de prise en charge thérapeutique.
Nous rapportons l’observation d’une patiente âgée de 57 ans, suivie depuis 19 ans pour un diabète de type 2, compliqué d’une rétinopathie et d’une neuropathie périphérique, admise pour une tuméfaction rouge et chaude du pied gauche suite à un traumatisme minime. Le bilan radiologique a objectivé une ostéolyse diffuse au niveau du Lisfranc, avec luxation du cuboïde. Il n’y avait pas de syndrome inflammatoire biologique, ni d’hyperuricémie. Les bilans rénal et lipidique étaient normaux. L’HbA1c était à 8,5%. L’éléctromyogramme a confirmé l’existence d’une neuropathie sensitivomotrice sévère.
La patiente a bénéficiée d’une insulinothérapie, d’une décharge à l’aide d’une botte plâtré avec béquille, relayée à la 8ème semaine par des chaussures orthopédiques. L’évolution était favorable. Cette observation pose le problème de diagnostic différentiel notamment avec l’ostéomyélite, tout en mettant l’accent sur la nécessité d’un diagnostic précoce et d’une prise en charge adaptée et prolongée.

Charcot’s foot : diagnostic and therapeutic dilemma

Charcot’s foot is a nervous arthropathy, that affects diabetic patients especially, and that raises the problem of diagno-sis and treatment.
We report the case of a 57-year-old woman, who was followed during 19 years for type 2 diabetes complicated by peripheral neuropathy and retinopathy, and who was hospitalized for erythematous and warm swelling of the left foot after a minor traumatism. Foot radiography revealed diffuse bone dissolution of the Lisfranc, and cuboide dislocation.
The erythrocyte sedimentation rate was not high. Uricema, renal function and lipid tests were normal. HBA1c was at 8,5%. Electomyography confirmed a severe sensitivo-motrice neuropathy. The treatment associated both insulinothe-rapy and relief from weight excess thanks to cast boot and crutches, relayed with orthopedic shoes after eight weeks. The evolution was favourable.
This observation emphasizes the problem of differential diagnosis particularly with osteomyelitis, the necessity for an early recognition of Charcot arthropathy and the adapted and prolonged care required in such cases.
