Objectifs : Evaluer la réponse virologique et la tolérance au traitement antiviral, chez des malades traités pour une hépatite virale C chronique, au service d’hépato-gastroentérologie du CHU Hassan II de Fès et de comparer nos résultats avec les données de la littérature nationale et internationale.
Malades et méthodes : Les dossiers de la totalité des malades traités dans le service d’hépato-gastroentérologie du CHU de Fès, pour hépatite chronique virale C, ont été analysés grâce à un logiciel d’analyse statistique (Epi-Info).
Résultats : Parmi les 59 malades traités pour une HVC (Hépatite Virus C), durant la période allant du 01/05/2001 au 01/10/2006, seulement 37 patients, naïfs, ont achevé leur traitement ainsi que leur suivi de 6 mois, après la fin du traitement. Leur âge moyen était de 59 ± 7,8 ans (41-72 ans), avec un sex-ratio (H/F) = 1,05. Dix-neuf malades (51%) avaient un génotype 1, et 18 patients (49%) avaient un génotype 2. La biopsie hépatique, réalisée chez 24 malades (65%), a montré une fibrose sévère chez 10 patients (42%). Sept patients (19%) avaient déjà une cirrhose constituée compensée. Vingt quatre malades (65%) ont reçu une bithérapie pégylée (Interféron pégylé associé à la ribavirine) et 13 patients (35%) ont reçu une bithérapie standard. Trois patients avaient arrêté le traitement précocement pour des effets secondaires. L’ARN du VHC était négatif en fin du traitement chez 19 malades (51%). Seize malades (43%) avaient une Réponse Virale Soutenue (RVS). En analyse univariée et multivariée l’âge et le génotype étaient des facteurs indépendants significativement associée à la RVS.
Conclusion : Ces résultats montrent que nos malades traités pour une hépatite virale C, ont un âge moyen de 59 ans, avec une répartition comparable du sexe. Le génotype 1 est relativement le plus fréquent (51%), avec une RVS de 43%, tous génotypes confondus.
Aims of the study: We report our experience in the treatment of chronic hepatitis C at the gastroenterology unit of the Fes University Hospital (Morocco) and compare our results with national and inter-national literature.
Patients and methods: The files of all patients treated for chronic hepatitis C in the gastro-enterology unit of the Fes University Hospital were analysed with Epi-Info software.
Results: Among 59 patients treated for VHC from 01/05/2001 to 01/10/2006, only 37 naive patients completed their treatment and their 6 months follow-up after the end of the treatment. Their mean age was 59 ± 7,8 years (41-72 years) with a sex-ratio (H/F) = 1,05. Nineteen patients (51%) had a genotype 1, and eighteen patients (49%) had a genotype 2. Ten (42%) patients had a severe fibrosis among 24 (65%) patients that had liver biopsy. Seven patients (19%) had compensated cirrhosis. Twenty four patients (65%) received pygelated bitherapy (pygelated interferon and ribavirin) and thirteen patients (35%) received a standard biotherapy (standard interferon and ribavirin). Three patients had stopped treatment prematurely for side effect. Viral RNA of hepatitis C was negative in the end of treatment in nineteen (51%) patients. The Sustained Viral Response (SVR), was 43%. In univariate and multivariate analysis age, and genotype were an independent factor significantly associated with SVR.
Conclusion: The mean age of our patients treated for chronic hepatitis C was 59 years old with equal spread for sex. Genotype 1 is relatively more frequent than others genotypes. Forty three percent treated patients had a SVR.
