Nous rapportons le cas d’un patient de 32 ans, qui présentait suite à une agression un couteau de cuisine fiché au niveau du 5EICG paravertébrale gauche.
A l’admission aux urgences chirurgicales, le patient était stable sur le plan hémodynamique, l’examen physique n’a pas objectivé de syndrome d’épanchement pleural ou péricardique. Le patient a été transféré en urgence au bloc opératoire pour thoracotomie gauche ; à l’exploration, la pointe du couteau était dans l’aorte thoracique entraînant une brèche sur la paroi de cette dernière ; l’ablation a été faite par manipulation douce après avoir mis des points au niveau de la brèche aortique. Le patient a été déclaré sortant à J2 d’hospitalisation, vu ultérieurement en consultation ; il ne présentait pas de complications.
Le but de ce travail est de signaler l’intérêt de faire l’extraction sous contrôle direct visuel par thoracotomie ou sternotomie d’un couteau surtout lorsqu’une plaie cardiaque ou des gros vaisseaux est suspectée.
We report the case of a patient of 32 years, which featured a kitchen knife driven into the left paravertebral 5EICG following a stabbing.
Admission to surgical emergency the patient was stable on the hemodynamic plan; the physical examination didn’t show a pleural or pericardial effusion syndrome. The patient was transferred immediately in the surgical unit for left thoracotomy; we noted during the exploration that the tip of the knife was in the thoracic aorta resulting a hole on the wall of the latter; removal was made by gentle manipulation after stitching at the level of the aortic gap. The patient was out 2 days after, and was later seen in consultation; He had no complications.
The purpose of this work is to report the interest to do extraction under direct view control by thoracotomy or sternotomy of a knife especially when a wound of heart or large vessels is suspected.
