Introduction : La localisation nasopharyngée du lymphome malin non-hodgkinien est une forme rare et inhabituelle contrairement au carcinome épidermoïde indifférencié du cavum. Il est responsable d’un problème de diagnostic positif clinique et histologique, d’où l’intérêt d’une étude immuno-histochimique.
Patients et méthodes : Nous rapportons, tout en analysant les résultats cliniques, thérapeutiques, ainsi que pronostiques, deux cas de lymphome malin non-hodgkinien du nasopharynx colligés dans le service d’ORL et de chirurgie cervico-faciale à l’hôpital 20 août du centre hospitalier universitaire Ibn Rochd Casablanca entre 2010 et 2013.
Observations : Le premier patient était de sexe masculin et le second du sexe féminin, âgés respectivement de 58 ans et 54 ans. Les signes cliniques étaient l’obstruction nasale, l’hypoacousie et les adénopathies cervicales. Le diagnostic était confirmé par la biopsie avec une étude immuno-histochimique. Les deux patients présentaient un lymphome malin non-hodgkinien de phénotype B à grandes cellules. Le traitement consistait en une chimiothérapie associée à une radiothérapie. L’évolution était marquée par une rémission complète dans les deux cas.
Conclusion : Le lymphome non-hodgkinien primitif du cavum est une entité rare et demeure de bon pronostic surtout le phénotype B. Le traitement optimal est une association de chimiothérapie et de radiothérapie. Un suivi clinique, endoscopique et radiologique est indispensable.
Introduction: Nasopharyngeal location of non-Hodgkin lymphoma is a rare and unusual form unlike undifferentiated squamous nasopharyngeal carcinoma. It is responsible for a problem of positive clinical and histological diagnosis, where the interest of an immune histochemical study.
Patients and methods: We report, analyzing clinical, therapeutic and prognostic outcomes, two cases of non-Hodgkin's lymphoma of the nasopharynx collected in the ENT department and Neck Surgery at the hospital 20 August at the University Hospital Ibn Rochd Casablanca between 2010 and 2013.
Observations: The first patient was male and the second one female, aged respectively 58 and 54 years. The clinical signs were nasal obstruction, hearing loss and cervical nodes. Diagnosis was confirmed by biopsy and immunohistochemistry study. Both patients had non-Hodgkin's lymphoma large B-cell phenotype. The treatment consisted of chemotherapy associated with radiotherapy. Evolution was characterized by a complete remission in both cases.
Conclusion: Primary non-Hodgkin lymphoma of the nasopharynx is a rare entity and remains good prognosis especially the B phenotype. The optimal treatment is a combination of chemotherapy and radiotherapy. Clinical, endoscopic and radiological follow-up is essential.
