Problématique : Le carcinome thyroïdien de l’enfant est une affection rare. Il se caractérise par un profil clinique et évolutif particulier et une prise en charge controversée. L’objectif de notre travail était d’étudier ces particularités cliniques et évolutives afin de déduire une approche thérapeutique appropriée.
Patients et méthodes : Nous avons mené une étude rétrospective portant sur 11 enfants atteints d’un carcinome thyroïdien suivis entre 2000 et 2011.
Résultats : Une prédominance féminine a été notée (7 filles/4 garçons). L’âge moyen était de 14 ans. Le mode révélateur était un nodule thyroïdien dans 54.54% des cas, un goitre multinodulaire dans 36.36% des cas et une adénopathie cervicale isolée dans un cas. Tous les patients ont eu une échographie cervicale et un seul patient a eu une cytoponction pour une adénopathie. Une thyroïdectomie totale et un évidement ganglionnaire central étaient pratiqués dans tous les cas. Neuf enfants étaient classés stade I et les 2 autres stade II. Les 10 patients atteints de carcinome différencié ont reçu une irathérapie. La survie était de 100%. La rémission complète était obtenue dans 72,7% des cas.
Discussion : Le carcinome thyroïdien chez l’enfant est caractérisé par une tendance à l’extension loco-régionale et à distance, contrastant avec un pronostic favorable. Le traitement consiste en une thyroïdectomie totale avec un évidement ganglionnaire adapté à l’extension de la maladie. La place de l’irathérapie est controversée.
Conclusion : La prise en charge des carcinomes thyroïdiens doit être adaptée à l’extension de la maladie sans exposer l’enfant à une morbidité accrue.
Background: Thyroid carcinoma in children is a rare condition. Its clinical and evolution course are different from those of the adult and its management is controversial. The purpose of this study was to evaluate clinical characteristics, course and outcome of thyroid carcinoma in children to deduce an appropriate therapeutic approach.
Patients and methods: Our study is retrospective concerning 11 children with thyroid carcinoma followed between 1996 and 2011.
Results: A female predominance was noted (7 girls /4 boys). The mean age was 14 years. Patients presented with thyroid nodule in 54.54% of cases, multinodular goiter in 36.36% of cases and isolated cervical lymph node in one case. All patients underwent neck ultrasonography and one patient had a lymph node fine needle aspiration. Total thyroidectomy and central neck dissection were performed in all cases. Nine children were classified as stage I and two as stage II. The ten patients with differentiated carcinoma received radioactive iodine therapy. The survival rate was 100%. Complete remission was achieved in 72,7% of patients.
Discussion: Thyroid carcinoma in children is characterized by a tendency for locoregional and metastatic spread, contrasting with a favorable prognosis. The treatment consists of a total thyroidectomy with lymph node dissection. The indication of radioactive iodine therapy is still controversial.
Conclusion: The management of thyroid carcinoma must be adapted to the spread of the disease without exposing the child to increased morbidity.
