Introduction : Actuellement les bêtabloquants doivent être introduits avant la sortie d’hospitalisation dans l’insuffisance cardiaque avec dysfonction systolique. Notre objectif était d’évaluer la fréquence d’utilisation des bêtabloquants dans l’insuffisance cardiaque au sein du CHU-JRB.
Méthode : Nous avons effectué une étude rétrospective descriptive transversale en 2011-2012 dans les services de Médecine Adulte du CHU-JRB, sur les dossiers des insuffisants cardiaques hospitalisés. Nous avons étudié l’âge, le genre, le(s) diagnostic(s), la fréquence cardiaque (FC), la pression artérielle systolique (PAS), la fraction d’éjection (FE), les comorbidités et le traitement de sortie des patients.
Résultats : Nous avons retenu 218 patients âgés de 14-98 ans (âge moyen = 55,08 ans) dont 104 (47,70%) hommes (sex-ratio = 0,91), hospitalisés durant 1-68 jours (moyenne = 10,48). Prescription de bêtabloquant chez n = 135 (61,9%) : métoprolol (n = 120), aténolol (n = 11), bisoprolol (n = 3) et propranolol (n = 1). La FC de sortie était de 44-120 bpm sans bêtabloquant (n = 83) et > 70 bpm chez 77 de ces patients (92,78%). L’échocardiographie a été faite avant la sortie chez 120 patients (55,04%) ; 86 patients (71,66%) avaient une FE ≤ 40% et 68,60% d’entre eux (n = 59) ont reçu un bêtabloquant ; 34 patients avaient une FE > 40% et 61,76% d’entre eux (n = 18) ont reçu un bêtabloquant. Chez les patients ayant en même temps une FE ≤ 40%, une PAS > 85 mmHg et une FC > 60 bpm, 24/80 (30%) ne recevaient pas de bêtabloquant. Les bêtabloquants étaient plus prescrits dans les services cardiologiques que dans les autres services de médecine (70,94% vs 51,48% ; p < 0,01).
Conclusion : Le taux d’utilisation des bêtabloquants était encore bas même en cas de FC élevée et/ou de FE diminuée, principales indications à ce traitement salvateur. Certains des médicaments utilisés n’avaient pas d’indication dans l’insuffisance cardiaque.
Introduction: Currently beta-blockers should be introduced prior to hospital discharge in heart failure patients with systolic dysfunction. We aimed to assess the prescription rate of beta-blockers in these patients in JRB teaching hospital.
Method: We performed a retrospective cross-sectional study in 2011-2012 in medical departments in the teaching hospital, on records of hospitalized heart failure patients. We studied the age, gender, diagnosis(s), heart rate (HR), systolic blood pressure (SBP), ejection fraction (EF), comorbidities and treatment at discharge.
Results: We enrolled 218 patients. The mean age was 55.08 years old (14-98), including 104 (47.70%) men (sex-ratio = 0.91), hospitalized for 1-68 days (mean = 10.48 days). Prescription of beta-blockers in n = 135 (61.9%): metoprolol (n = 120), atenolol (n = 11), bisoprolol (n = 3) and propranolol (n = 1). HR at discharge ranged from 44 to 120 bpm without beta-blocker (n = 83) and was > 70 bpm in 77 of these patients (92.78%). Echocardiography was performed in 120 patients (55.04%); 86 patients (71.66%) had an EF ≤ 40% and 68.60% of them (n = 59) received a beta-blocker; 34 patients had an EF > 40% and 61.76% of them (n = 18) received a beta-blocker. Among patients with FE ≤ 40%, SBP > 85 mmHg and FC > 60 bpm, n = 24/80 (30%) did not receive any beta-blocker. Beta-blockers were more prescribed in cardiology departments than in other departments (70.94% vs. 51.48%; p < 0.01).
Conclusion: The prescription rate of beta-blockers was low even among patients with high heart rate and/or reduced ejection fraction. Some of the prescribed drugs were not indicated in heart failure.
