Introduction : Non-traitées, les pathologies vitréo-rétiniennes entraînent une cécité irréversible et définitive. Pourtant, la chirurgie vitréo-rétinienne n’existe pas encore à Madagascar. L’objectif de notre étude est de déterminer les aspects épidémio-cliniques des pathologies vitréo-rétiniennes nécessitant une prise en charge chirurgicale, hospitalisées au service d’ophtalmologie du CHUA-HJRA.
Méthodes : Etude rétrospective descriptive chez les patients présentant des pathologies chirurgicales vitréo-rétiniennes, hospitalisés au service d’ophtalmologie du CHUA-HJRA, pendant la période de janvier 2009 à août 2013.
Résultats : Nous avons recensé 31 cas sur les 2216 patients hospitalisés, soit 1,40%. L’âge moyen (± ET) était de 34,93 ans (± 16,27). Les traumatismes du segment postérieur prédominaient avec 27 cas soit 87,10%, contre 4 cas d’endophtalmie (12,90%). Parmi les traumatismes, les circonstances de l’accident étaient dominées par les accidents du travail à 11 cas (37,03%). Une hémorragie du vitré était présente dans tous les cas de traumatismes ; nous avons trouvé 16 cas de décollement de rétine (59,26%) et 8 cas de corps étranger dans le vitré (29,63%). Aucun de nos patients n’a bénéficié d’une vitrectomie. Parmi les traumatismes, les acuités visuelles initiales étaient toutes inférieures à "compte les doigts" ; en fin d’évolution, nous n’avons trouvé qu’une seule acuité visuelle à 4/10ème, le reste est resté à inférieure "compte les doigts". Parmi les 4 cas d’endophtalmies, 3 yeux ont été l’objet d’une éviscération.
Conclusion : Les traumatismes constituent les principales pathologies chirurgicales vitréo-rétiniennes vues en hospitalisation au service d’ophtalmologie du CHUA-HJRA. Les circonstances de l’accident sont dominées par les accidents du travail. L’absence de la vitrectomie est responsable d’une évolution médiocre.
Introduction: Untreated vitreoretinal pathologies can lead to irreversible and definitive blindness. However vitreoretinal surgery doesn’t exist yet in Madagascar. This study aims to assess the clinical and epidemiological profile of vitreoretinal pathologies which need surgical intervention at the ophthalmology unit of the teaching hospital of Joseph Ravoahangy Andrianavalona.
Methods: A retrospective descriptive study was carried out during a period of three years and eight months, from January to 2009 to August 2013. Patients with surgical vitreoretinal pathologies admitted at the ophthalmology unit of the teaching hospital of Joseph Ravoahangy Andrianavalona during the study period were included.
Results: In total, 31 cases (1,40%) of vitreoretinal pathologies among 2216 hospitalized patients were recorded. The mean age (± SD) was evaluated to 34,93 years (± 16,27). Posterior segment trauma was the most represented case with a proportion of 87,10% (27 cases). Endophtalmitis represented 12,90% of the case (4 cases). The circumstances of the accident of all injuries were dominated by work-related accident (37,03%). Vitreous hemorrhage was observed in all injuries cases. The proportion of retinal detachment and vitreal foreign object were respectively estimated to 59,26% (16 cases) and 29,63% (8 cases). None of our patients had vitrectomy. Initial visual acuity of all patients with injuries was lower than “counts fingers”. At the end of the follow-up, only one patient had visual acuity of 4/10. The visual acuity of the rest of the patient didn’t change. Three of the four endophtalmitis cases had ocular evisceration.
Conclusion: Injuries represent the main vitreoretinal pathologies observed among patients hospitalized at the ophthalmology unit of the teaching hospital of JRA. Accident circumstances were dominated by work-related injuries. Absence of vitrectomy was responsible of bad evolution of cases.
