Introduction : La hernie diaphragmatique droite est l’une des hernies rarement diagnostiquées en pratique courante. Elle représente 20% des hernies diaphragmatiques. C’est une urgence néonatale, le diagnostic passe souvent inaperçu et se révèle par des complications. Très peu de cas ont été listés dans la littérature, raison pour laquelle, nous rapportons le cas d’une hernie diaphragmatique droite à révélation tardive chez un nourrisson de quarante jours.
Observation : Le patient a été reçu pour détresse respiratoire, chez qui l’examen clinique avait retrouvé un syndrome de condensation pulmonaire gauche. La radiographie cur/poumon montrait la présence de clartés digestives dans l’hémithorax droit associée à une opacité à la base pulmonaire droite correspondant au foie. Le scanner thoracique montrait une hernie diaphragmatique droite, avec passage du côlon et du foie en intra-thoracique. Le patient a été mis sous oxygénothérapie et antibiothérapie, puis a été pris en charge sur le plan chirurgical sans complications notées.
Conclusion : Devant toute symptomatologie respiratoire et ou digestive atypique chez un nourrisson, penser à une hernie diaphragmatique jusqu’à preuve du contraire. La prise en charge chirurgicale reste la pierre angulaire dans la majorité des cas, elle améliore le pronostique du patient, tout en limitant le risque de séquelles.
Introduction: The Right Diaphragmatic Hernia (RDH) is one of the rarely diagnosed hernias in current practice. It represents 20% of diaphragmatic hernias. It is a neonatal emergency; the diagnosis often goes unnoticed and is revealed by complications. Very few cases have been listed in the literature, so we report the case of right diaphragmatic hernia of late revelation in a 40-day-old infant.
Observation: The patient was received for respiratory distress, in whom the clinical examination found a left pulmonary condensation syndrome. The heart/lung radiograph showed the presence of digestive clarity in the right hemi-thorax associated with opacity at the right pulmonary base corresponding to the liver. The chest CT showed a right diaphragmatic hernia, with passage of the colon and liver into the intrathoracic area. The patient was put on oxygen and antibiotherapy, then was treated surgically without any noted complications.
Conclusion: Faced with any respiratory and/or atypical digestive system symptomatology in an infant, think of a diaphragmatic hernia until proven otherwise. Surgical management remains the cornerstone in the majority of cases; it improves the prognosis of the patient, while limiting the risk of sequelae.
