Les patients en hyperthyroïdie sont exposés à un risque thromboembolique élevé, et à des complications cardiaques rythmiques particulières. Nous rapportant deux cas de cardiothyréose associés à une suspicion d’embolie pulmonaire. La première patiente avait 58 ans et la deuxième 78 ans. Le diagnostic de cardiothyréose a été posé devant l’association d’hyperthyroïdie et trouble de rythme cardiaque à type de fibrillation atriale, d’extrasystole ventriculaire et de tachycardie ventriculaire. L'embolie pulmonaire était évoquée devant un tableau d’insuffisance cardiaque droite avec hypertension artérielle pulmonaire d’allure précapillaire et une élévation importante des D-dimères pour les deux patientes. Malgré une bonne anticoagulation, un traitement antiarythmique et antithyroïdien de synthèse optimal, ces deux patientes étaient décédées. La première avait présenté une mort subite sur tachycardie ventriculaire et la deuxième était décédée dans un tableau d’insuffisance cardiaque droite sévère avec dégradation de l’état hémodynamique. Notre objectif était de décrire la difficulté de prise en charge de la cardiothyréose sur le plan rythmique et sur le plan thromboembolique.
Patients with hyperthyroidism are exposed to a high thromboembolic risk, and a specific rhythmic complication concerning the heart. We report two cases of cardiothyreosis associated with a probable pulmonary embolism. The first patient was 58 years old and the second 78 years old. The diagnosis of cardiothyreosis was recalled in front of hyperthyroidism with a rhythm disorder such as atrial fibrillation, ventricular extrasystole and ventricular tachycardia. Pulmonary embolism was evoked in front of a right heart failure with precapillary pulmonary hypertension and a significant elevation of D-dimer for both patients. Despite good anticoagulation, optimal antiarrhythmic and antithyroid treatment, these two patients had died. The first had presented a sudden death from ventricular tachycardia and the second had died in a severe right heart failure with deterioration of the hemodynamic state. Our goal was to describe the difficulty of taking care cardiothyreosis on a rhythmic and thromboembolic level.
