Introduction : La lombosciatique est une affection potentiellement handicapante dans toutes les tranches d’âge. Plusieurs études ont été consacrées à sa prise en charge chirurgicale, montrant ainsi que ce traitement se vulgarise sur le continent. Les critères d’indications entre le traitement chirurgical et le traitement conservateur peuvent dans certains cas être objet de discussion du fait de la multidisciplinarité de cette affection. Nous avons rapporté notre expérience et les difficultés de prise en charge des lombosciatiques à Cotonou et analyser l’impact du traitement chirurgical.
Patients et méthodes : Il s’agit d’une étude rétrospective, à visée descriptive et exhaustive, ayant porté sur la période allant du 1er janvier 2012 au 31 décembre 2017 à Cotonou. La variable dépendante était l’évolution de la lombosciatique et le déficit neurologique 6 mois après leur traitement conservateur ou chirurgical.
Résultats : L’âge moyen des patients était de 51.10 ± 12.29 ans avec des extrêmes allant de 19 à 82 ans. Le sex-ratio était de 0.83. L’évolution de la douleur a été favorable pour 88,9% des patients opérés. La comparaison des deux groupes montrait que le repos (p = 0,008), et le traitement chirurgical (p = 0,000) était des facteurs pouvant influencer l’évolution de la douleur.
Conclusion : La lombosciatique est une affection touchant les adultes surtout à partir de la 4ème décade. Le traitement chirurgical malgré son efficacité sur la douleur, suscite encore la hantise chez les patients.
Introduction: Low back pain and radiculopathy is a potentially disabling condition in all age groups. Several studies have been devoted to its surgical management, showing that this treatment is becoming more widespread on the continent. The criteria for indications between surgical and conservative treatment can in some cases be the subject of discussion due to the multidisciplinary nature of this condition. We have reported our experience and the difficulties in the management of Low back pain and radiculopathy in Cotonou and analyze the impact of surgical treatment.
Patients and methods: This is a retrospective study, descriptive and exhaustive, covering the period from January 1st, 2012 to December 31st, 2017 in Cotonou. The dependent variable was the evolution of low back pain and radiculopathy and neurological deficit 6 months after their conservative or surgical treatment.
Results: The average age of the patients was 51.10 ± 12.29 years with extremes ranging from 19 to 82 years. The sex ratio was 0.83. The evolution of pain was favorable for 88.9% of the patients operated. The comparison of the two groups showed that rest (p = 0.008) and surgical treatment (p = 0.000) were factors that could influence the evolution of pain.
Conclusion: Low back pain and radiculopathy is a condition affecting adults especially from the 4th decade onwards. Surgical treatment, despite its effectiveness on pain, is still haunting patients.
