Introduction : Le syndrome de Lemierre est un sepsis rare et grave, à point de départ oropharyngé. Il est caractérisé par une thrombophlébite de la veine jugulaire interne. C’est une pathologie potentiellement létale en l’absence d’une prise en charge précoce et adaptée.
Cas clinique : Nous rapportons un cas de syndrome de Lemierre faisant suite à une suppuration cervicale d’origine oropharyngée chez un patient de 30 ans. La tomodensitométrie cervico-faciale avait mis en évidence une thrombophlébite de la veine jugulaire interne. Aucun agent infectieux n’avait été isolé dans les prélèvements réalisés. Le traitement était avant tout médical associant une antibiothérapie et une héparinothérapie, mais aussi chirurgical consistant en un drainage des collections abcédées. Les suites étaient simples et l’évolution était favorable.
Discussion : Le syndrome de Lemierre est une affection grave et mortelle en recrudescence. Il reste cependant une pathologie méconnue et sous diagnostiquée. Il doit être évoqué devant toute tuméfaction cervicale douloureuse associée à une infection oropharyngée récente.
Introduction: Lemierre's syndrome is a rare and serious sepsis, with an oropharyngeal starting point. It is characterized by a thrombophlebitis of the internal jugular vein. It is a potentially fatal condition in the absence of early and appropriate management.
Clinical case: We report a case of Lemierre’s syndrome following cervical suppuration of oropharyngeal origin in a 30-year-old patient. Head and neck computed tomography revealed a thrombophlebitis of the internal jugular vein. No infectious agent had been isolated in the samples taken. The treatment was above all medical combining antibiotic therapy and heparin therapy, but also surgical, consisting of drainage of the abscessed collections. The suites were simple, and the course was favorable.
Discussion: Lemierre's syndrome is a serious and fatal disease on the rise. However, it remains an unknown and underdiagnosed pathology. It should be considered in the event of any painful cervical swelling associated with a recent oropharyngeal infection.
