Introduction : L’Infection Spontanée du Liquide d’Ascite (ISLA) est fréquente et grave chez les cirrhotiques. L’objectif de cette étude était de décrire les caractéristiques cliniques et bactériologiques de l’infection spontanée du liquide d’ascite à Madagascar.
Patients et méthodes : Il s’agit d’une étude prospective sur 21 mois entre janvier 2018 et octobre 2019, incluant des patients hospitalisés avec une cirrhose, présentant des symptômes clinico-biologiques en faveur d’une ISLA.
Résultats : Trente-trois patients ont été inclus. L’âge moyen était de 48,09 ± 13,55 ans (extrêmes : 19-78 ans), le sex-ratio était de 3,12. La douleur abdominale (55%), la fièvre (36%), la diarrhée (6%), l’encéphalopathie hépatique (18%) sont les principaux symptômes révélateurs. L’hémorragie digestive (18,18%) était le principal facteur favorisant de l’ISLA. L’ISLA était communautaire dans 87,88% des cas. Une culture de liquide d’ascite a été positive pour 9 patients (27,27%). Les germes retrouvés étaient Escherichia coli (12,10%), Klebsiella pneumoniae (3%), Pseudomonas (3%), Streptocoque mitis (9,1%). Les Escherichia coli étaient sauvages avec un cas résistant à la Ceftriaxone. Les Klebsiella étaient multirésistants. Les deux autres germes ne présentaient pas de résistance. Après une antibiothérapie adaptée à l’antibiogramme une guérison était observée chez 26 patients (78,78%). Sept patients (21,22%) sont décédés à la suite de diverses complications. Ils étaient tous associés à la présence de bactérie dans l’ascite.
Conclusion : L’ISLA définie selon des critères clinico-biologiques est apparemment stérile dans la majorité des cas. Les bactéries à Gram négatif étaient les principaux agents pathogènes impliqués dans l’ISLA. Escherichia coli et streptocoque étaient les agents pathogènes les plus couramment isolés. L’étude bactériologique est essentielle pour adapter l’antibiothérapie aux bactéries multirésistantes.
Introduction: Spontaneous bacterial peritonitis is common and serious in patients with cirrhosis. The aim of this study was to describe the clinical and bacteriological characteristics of Spontaneous bacterial peritonitis in Madagascar.
Patients and methods: This is a 21-month prospective study between January 2018 and October 2019, including hospitalized patients with cirrhosis, with clinical and biological symptoms ofspontaneous bacterial peritonitis.
Results: Thirty-three patients were included. The mean age was 48.09 ± 13.55 years (extremes: 19-78 years), the sex-ratio was 3.12. Abdominal pain (55%), fever (36%), diarrhea (6%), hepatic encephalopathy (18%) are the most common symptoms. Gastrointestinal bleeding (18.18%) was the main risk factor to SBP. SBP was community-acquired in 87.88% of cases. A culture of ascites fluid was positive for 9 patients (27.27%). The infectious agents found were Escherichia coli (12.10%), Klebsiella pneumoniae (3%), Pseudomonas (3%), Streptococcus mitis (9.1%). Escherichia coli were wild with one case resistant to Ceftriaxone. The Klebsiella were multidrug resistant. The other two pathogens did not show resistance. After antibiotic therapy adapted to the antibiogram, healing was observed in 26 patients (78.78%). Seven patients (21.22%) died from various complications. All deceased patients had bacteria identified in ascites fluid.
Conclusion: Spontaneous bacterial peritonitis defined according to clinical and biological criteria is apparently sterile in the majority of cases. Gram-negative bacteria were the major pathogens involved in spontaneous bacterial peritonitis in cirrhotic patients. Escherichia coli and Streptococcus were the most common pathogen isolated. Bacteriological study is essential to adapt antimicrobial to multidrug-resistant bacteria.
