Résumé

Objectif : Appréhender l’éventuel parallélisme entre détermination génétique HLA, maladie parodontale et polyarthrite rhumatoïde.
Matériels et méthodes : L’étude entre dans le cadre d’une étude cas-témoins rétrospective classique (sujets atteint de polyarthrite rhumatoïde versus patients présentant d’autres pathologies rhumatismales). Elle porte sur 32 sujets atteints de polyarthrite rhumatoïde (cas) et 61 sujets diagnostiqués non porteurs. Le diagnostic de la polyarthrite rhumatoïde a été réalisé par un médecin rhumatologue. Un typage génétique HLA a été effectué.
Résultats : Il existe une plus grande fréquence de parodontite établie chez les patients atteints de polyarthrite rhumatoïde avec p = 0,038. En l'absence de maladie parodontale, le Shared epitope est un facteur de risque de polyarthrite rhumatoïde avec OR = 14.032 [IC 95% : 3.774-52.164]. L’'interaction entre le nombre de sites présentant une perte d'attache ≥ 5 mm et le Shared epitope a un effet significatif sur la sécrétion d'anticorps anti-citrullinés (aCCP) avec p = 0,024.
Conclusion : Même en l'absence de maladie parodontale, le rôle du Shared epitope a été établi. L’interaction entre perte d'attache/Shared epitope influence la sécrétion d'anticorps anti-citrullinés.

Abstract
Relationships between rheumatoid arthritis, periodontal disease and Shared epitope

Aim: It was principally a better understanding between the interactive role of Shared epitope, periodontal disease and in the course of rheumatoid arthritis.
Materials and methods: A case control stud on 32 subjects with rheumatoid arthritis (cases) and 61 subjects diagnosed as non-carrier. Diagnosis of rheumatoid arthritis state was performed by a specialist physician. A genetic HLA typing was done.
Results: A greater frequency of established periodontitis in the patients with rheumatoid arthritis p = 0,038. In the absence of periodontal disease, the shared epitope is a risk factor for rheumatoid arthritis with OR = 14.032 [IC 95%: 3.774-52.164]. The interaction between number of sites with attachment loss ≥ 5 mm and Shared epitope has a significant effect on the secretion of anti-citrullinated protein antibodies, p = 0.024.
Conclusion: Even in the absence of periodontal disease, the role of Shared epitope was established. The interaction attachment loss/ Shared epitope influences the secretion of anti-citrullinated protein antibodies.
