Objectifs : Comparer l'efficacité anesthésique de l'articaïne à 4% avec l'épinéphrine au 1/100.000 et la lidocaïne à 2% avec l'épinéphrine à 1/80.000 dans la chirurgie de la troisième molaire mandibulaire.
Matériels et méthodes : Essai clinique randomisé en double aveugle dans lequel 28 patients avec indication de chirurgie de troisième molaire ont reçu un bloc mandibulaire conventionnel avec 4% d'articaïne avec épinéphrine 1/100.000 et 2% de lidocaïne avec épinéphrine 1: 80.000 dans différents rendez-vous. Le degré d'anesthésie pulpaire a été évalué avec un testeur de pulpe électronique, le besoin d'anesthésie complémentaire pendant la chirurgie et l'anesthésie des tissus mous ont été enregistrés ainsi que les complications opératoires et anesthésiques associées. Les données ont été évaluées en utilisant un test de Chi carré et un test de Student.
Résultats : L'anesthésie pulpaire dans les molaires mandibulaires a été couronnée de succès avec 14,29% de lidocaïne et 7,14% avec de l'articaïne. Une technique anesthésique complémentaire a été utilisée dans 41,07%, en utilisant la technique intra-ligamentaire dans 69,57% au cours de la luxation, sans aucune différence dans l'anesthésique utilisé. L'anesthésie des tissus mous a été réussie dans la gencive avec 91,07% et 89,29% à 4 et 45 minutes respectivement, dans la lèvre inférieure, le taux de succès était de 58,93%. Des complications peropératoires sont survenues chez 1,78% des patients. Aucune complication liée à l'anesthésie n'a été rapportée.
Conclusion : Il n'y avait pas de différences statistiquement significatives entre les médicaments utilisés pour une anesthésie pulpaire réussie après l'utilisation de la technique mandibulaire conventionnelle.
Aims: To compare the anesthetic efficacy of 4% articaine with epinephrine 1:100,000 and 2% lidocaine with epinephrine 1:80,000 in mandibular third molar surgery.
Materials and methods: Randomized, double-blind clinical trial in which 28 patients with indication of third molar surgery received a conventional mandibular block with 4% articaine with epinephrine 1:100,000 and 2% lidocaine with epinephrine 1: 80,000 in different appointments. The degree of pulpal anesthesia was evaluated with an electronic pulp tester, the need for complementary anesthesia during surgery and soft tissue anesthesia were recorded as well as the operative and anesthetic-associated complications. The data was evaluated using a chi square test and Student's test.
Results: Pulpal anesthesia in mandibular molars was successful in 14.29% with lidocaine and 7.14% with articaine. A complementary anesthetic technique was used in 41.07%, using the intra-ligamentary technique in 69.57% during the dislocation, with no differences in the anesthetic used. Soft tissue anesthesia was successful in gingiva with 91.07% and 89.29% at 4 and 45 minutes respectively, in the lower lip the success rate was 58.93%. Intraoperative complications occurred in 1.78%. No complications related to the anesthetic were reported.
Conclusion: There were no statistically significant differences between the drugs used for successful pulpal anesthesia after using the conventional mandibular technique.
