Résumé

Contexte : L’allongement de la durée de la vie et les progrès de la médecine ont pour contrepartie une augmentation de la prévalence des patients à risque infectieux, susceptibles de recevoir un traitement endodontique. La pratique de l’endodontie ne peut se concevoir en dehors du contexte médical du patient.
Objectif : L’objectif de cette étude était d’évaluer les connaissances des praticiens des Centres Hospitaliers Universitaires (CHU) d’Abidjan, sur les recommandations de l’Afssaps 2011 [1] , en matière de prise en charge des patients cardiopathes lors du Traitement Endodontique (TE).
Matériel et méthode : Il s’agit d’une enquête transversale à visée descriptive qui a consisté à interroger 145 chirurgiens-dentistes. Les questions ont été orientées sur les différents types de cardiopathies et les indications ou contre-indications de la thérapeutique endodontique pour le traitement des pulpopathies irréversibles.
Résultats : Selon notre échantillon, plus de 50% des praticiens connaissent parfaitement les mesures d’indications et de contre-indications du traitement endodontique chez le cardiopathe. Lorsque le traitement endodontique est indiqué, certains praticiens ont eu du mal à traiter la dent en une séance conformément aux recommandations de l’Afssaps 2011. Deux fois sur 3, l’antibioprophylaxie n’a pas été prescrite 1h avant le soin (75,17%). Enfin, 26,21%, soit 1 chirurgien-dentiste sur 3 interrogés n’a pas trouvé nécessaire d’établir le dialogue avec le médecin traitant.
Conclusion :Notre étude a révélé que très peu de praticiens appliquent les directives actuelles en matière de prise en charge des patients cardiopathes. Ce qui traduit le besoin de la mise en place au sein des CHU d'Abidjan, de médecins-dentistes référents habilités à assurer la prise en charge des patients cardiopathes dont l’état nécessite un TE. En effet cette activité mérite une expertise. Aussi, il est important que la collaboration et une communication entre le service d'endodontie des CHU et des cardiologues soient renforcées. Le cahier de charge de cette prise en charge de cette spécialité mérite d’être établie et mise en œuvre au bénéfice de tous les patients à risques de nos 3 services hospitalo-universitaires.

Abstract
Are the recommendations for oral and endodontic cardiac management strictly adhered to by the three University Hospitals of Abidjan?

Background: The lengthening of life and the progress of medicine have as a counterpart an increase in the prevalence of patients at risk of infection, likely to receive endodontic treatment. The practice of endodontics cannot be conceived outside the patient’s medical context.
Objective: The objective of this study was to assess the knowledge of practitioners from the Abidjan teaching hospitals on current guidelines [1]. for the management of heart disease patients during Endodontic Treatment (ET).
Material and method: This is a cross-sectional descriptive survey which consisted of interviewing 145 dentists. The questions were oriented on the different types of heart disease and the indications or contraindications of endodontic therapy for the treatment of irreversible pulpopathies.
Results: More than 50% of practitioners are fully aware of the measures of indications and contraindications for endodontic treatment in cardiac patients. When endodontic treatment is indicated, some practitioners have had difficulty treating the tooth in one session as indicated in the last recommendations. However, more than 1/3 easily passed TE in one time and, 2 times out of 3 antibiotic prophylaxis was prescribed 1 hour before treatment (75.17%). Finally, 26.21%, 1 in 3 dental surgeons interviewed did not find it necessary to establish a dialogue with the attending physician.
Conclusion: Our study found that very few practitioners follow current guidelines for the management of cardiac patients. This reflects the need to set up within the Abidjan University Hospitals (CHU), referring dentists qualified to ensure the management of cardiopathic patients whose condition requires TE. Indeed, this activity deserves an expertise. Also, it is important that collaboration and communication between the endodontic department of the CHU and cardiologists be strengthened. The specifications for the management of this specialty deserve to be established and implemented for the benefit of all patients at risk in our 3 university hospital departments.
