Les ruptures du tendon distal du Muscle Grand Pectoral (MGP) sont rares. Dans la littérature, on retrouve environ 200 cas de rupture traumatique du MGP. Les circonstances de survenue, la présentation clinique et l’anatomopathologie ont été bien décrites. A l’inverse, la littérature de la rupture traumatique du MGP par mécanisme direct, au niveau de son ventre, est peu fournie, voire inexistante. Cette rupture traumatique du MGP associée à une plaie vasculaire, présente un tableau clinique inhabituel. Nous présentons un cas de rupture traumatique du MGP avec atteinte du pédicule vasculo-nerveux axillaire, chez un adolescent de 17 ans. Le tableau clinique a fait évoquer un hématome comprimant le paquet vasculo-nerveux axillaire. L’ischémie aiguë du membre et une augmentation progressive du volume de l’hématome ont été les arguments majeurs de l’indication d’une exploration en urgence, en vue de lever la compression de l’artère axillaire et réaliser l’hémostase. La rupture traumatique par mécanisme direct du MGP au niveau de son ventre, est une urgence rare et grave lorsqu’elle est associée à une atteinte du pédicule vasculo-nerveux axillaire. L’atteinte de ce dernier doit être rapidement éliminée cliniquement, tandis qu’une échographie confirmera la lésion du MGP. Cependant l’imagerie ne doit pas retarder la prise en charge.
Ruptures of the distal tendon of the Pectoralis Major Muscle (PMM) are rare. In the literature, there are about 200 cases of traumatic PMM rupture. The circumstances of occurrence, clinical presentation and pathology have been well described. Conversely, the literature of the traumatic rupture of PMM by direct mechanism, at the level of its belly, is little provided, or even non-existent. This traumatic rupture of PMM associated with a vascular wound presents an unusual clinical picture. We present a case of traumatic rupture of PMM with involvement of the axillary neurovascular pedicle in a 17-year-old adolescent. The clinical presentation evoked a hematoma compressing the axillary vasculo-neural bundle. The acute ischemia of the limb and a progressive increase of the volume of the hematoma were the major arguments of the indication of an emergency exploration, with a view to lifting axillary artery compression and achieving hemostasis. The traumatic rupture by direct mechanism of the PMM at the level of its belly, is a rare and serious emergency when it is associated with a lesion of the axillary vasculo-nervous pedicle. The lesion of the latter must be promptly eliminated clinically, while an ultrasound will confirm the lesion of PMM. However, imaging should not delay management.
