Introduction : La mortalité par Syndrome Coronarien Aigu (SCA) des sujets âgés et les facteurs associés ne sont pas connus en Afrique subsaharienne. Notre but était d’évaluer la mortalité du SCA chez les sujets âgés et en déterminer les facteurs prédictifs à Ouagadougou.
Patients et méthodes : Une étude rétrospective multicentrique analytique a porté sur les patients consécutifs âgés de 60 ans et plus hospitalisés de janvier 2017 à septembre 2019 pour un SCA à Ouagadougou. Les caractéristiques sociodémographiques, cliniques, paracliniques et le traitement ont été recueillis. L’analyse statistique a été réalisée grâce au logiciel SPSS 20 avec un seuil de signification p < 5% pour un intervalle de confiance à 95%.
Résultats : Quatre-vingt-huit patients d’âge moyen de 70,77 ± 8,9 ans ont été inclus. Le SCA avec sus-décalage du segment ST représentait 55,7% des cas. Le taux de mortalité intra-hospitalier a été de 22,7%. Deux facteurs indépendants de mortalité ont été identifiés l’élévation de la troponine et la non mise en route d’un traitement médicamenteux conventionnel du SCA. En analyse univariée, les principaux facteurs associés au décès étaient le long délai de prise en charge, le SCAST+, la classe KILLIP ≥ 2, une fraction d’éjection du ventricule gauche < 50% et un syndrome inflammatoire.
Conclusion : La mortalité intra-hospitalière du SCA chez le sujet de 60 ans et plus est élevée. Une prise en charge précoce avec un traitement médical optimal et la revascularisation en amélioreraient le pronostic.
Introduction: Acute Coronary Syndrome (ACS) mortality in the elderly and associated factors are not known in sub-Saharan Africa. Our purpose was to assess mortality of ACS in the elderly and determine the predictive factors in Ouagadougou.
Patients and methods: An analytical multicenter retrospective study was performed on consecutive patients aged 60 years and over hospitalized from January 2017 to September 2019 for an ACS in Ouagadougou. Socio-demographic, clinical, paraclinical and treatment characteristics were collected. Statistical analysis was performed using SPSS 20 software with a significance level p < 5% for a 95% confidence interval.
Results: Eighty-eight patients with an average age of 70.77 ± 8.9 years were included. The ST segment elevation ACS represented 55.7% of cases. The in-hospital mortality rate was 22.7%. Two independent factors of mortality have been identified, the elevation of troponin and the failure to initiate conventional drug therapy for ACS. In univariate analysis, the main factors associated with death were the long delay to care, SCAST +, KILLIP class ≥ 2, left ventricle ejection fraction < 50% and an inflammatory syndrome.
Conclusion: Intra hospital mortality of ACS in subjects aged to 60 years and older is high. Early management with optimal medical treatment and revascularization would improve the prognosis.
