Résumé

Introduction : Le but de notre travail était d’évaluer la fonction érectile de patients opérés pour un hypospadias à l’âge adulte dans notre centre.
Patients et méthode : Il s’agissait d’une étude rétrospective, descriptive et monocentrique, colligeant les dossiers de patients opérés pour hypospadias à l’âge adulte dans le service d’urologie de l’Hôpital Aristide Le Dantec entre janvier 2010 et décembre 2016. Les paramètres étudiés ont été : l’âge, leur situation matrimoniale, les antécédents, le type d’hypospadias et l’évaluation de la fonction sexuelle par le questionnaire IIEF 15 après la cure.
Résultats : Sur une période de 6 ans, 16 patients ont été opérés pour un hypospadias à l’âge adulte. L’âge moyen était de 24,75 ± 7,58 ans. Les hypospadias postérieurs étaient dominants (9 cas). Cinq patients n’avaient jamais eu de rapports sexuels. Ainsi le questionnaire IIEF 15 n’a été administré qu’à 11 patients, parmi eux quatre n’avaient pas de trouble de la fonction érectile, Six avaient une dysfonction érectile peu sévère et un patient avait, une dysfonction moyennement sévère. Dix patients avaient une fonction orgasmique normale et un patient avait un trouble de la fonction orgasmique. Neuf patients avaient une libido normale. Sept patients étaient satisfaits sexuellement, un l’était moyennement et 3 patients étaient insatisfaits. Neuf patients avaient une satisfaction générale. Sept patients avaient une très bonne acceptation de leur morphologie et 7 autres une bonne acceptation. Deux patients ayant un hypospadias postérieur avaient des troubles de l’éjaculation. Ils étaient mariés sans enfants. Quatorze patients auraient préféré être opérés durant l’enfance alors que les deux autres étaient neutres. Sur les 7 hypospadias postérieurs 5 étaient satisfaits sexuellement, un patient l’était moyennement et un patient ne l’était pas.
Conclusion : La majeure partie des patients avait une bonne acceptation morphologique et une satisfaction sur le plan sexuel même dans les hypospadias postérieurs.

Evaluation of the erectile function of patients operated on for hypospadias in adulthood

Introduction: The purpose of our work was to study the erectile function of patients operated for hypospadias in adulthood in our center.
Patients and methods: This was a retrospective, descriptive study, single center over a period of 6 years, compiling all records of patients operated on for hypospadias as an adult in the urology
department of Hospital Aristide DANTEC between January 2010 and December 2016. The parameters studied were: age, their marital status, the type of hypospadias and the IIEF 15 questionnaire after the treatment.
Results: Over a period of 6 years, sixteen patients operated for hypospadias in adulthood were identified with an average age of 24.75 ± 7.58 years. In our series the posterior hypospadias were dominant (9 cases). Five patients had never had sex. Thus, the IIEF 15 questionnaire was only asked to 11 patients. Of these patients, four had no erectile dysfunction, six had mild erectile dysfunction, and one had mild to moderate dysfunction. Ten patients had normal orgasmic function. Nine patients had a normal libido and two had libido disorders. Seven patients were satisfied sexually, one was moderately, and 3 patients were dissatisfied. Nine patients had general satisfaction. Seven patients had a very good acceptance of their morphology and 7 others a good acceptance. Two patients with posterior hypospadias had ejaculation disorders. They were married without children. Fourteen patients would have preferred surgery during childhood while the other two were neutral. On 7 posterior hypospadias, 5 were sexually satisfied, one was moderate and was not.
Conclusion: The majority of patients had good morphological acceptance and sexual satisfaction even in posterior hypospadias.
