Introduction : Les tumeurs cérébrales constituent un ensemble de néoformations intracrâniennes. Leur symptomatologie associe un syndrome d’Hypertension Intra-Crânienne (HTIC) et un syndrome neurologique focal. La tomodensitométrie cérébrale et l’imagerie par résonnance magnétique sont d’un grand apport diagnostique. L’objectif de notre travail était de déterminer les aspects épidémiologiques, cliniques et diagnostiques des tumeurs cérébrales des enfants hospitalisés au Centre Hospitalier National d’Enfants Albert Royer (CHNEAR) de Dakar.
Matériel et méthodes : Il s’agissait d’une étude descriptive des tumeurs cérébrales chez les enfants hospitalisés au service des urgences du centre hospitalier national d’enfants Albert Royer du 1er janvier 2005 au 31 décembre 2014 (10 ans). Les données ont été recueillies à partir des dossiers des patients et des registres d’hospitalisation.
Résultats : Trente patients présentaient une tumeur cérébrale, 16 ont été définitivement inclus dans l’étude soit une incidence de 0,07%. L’âge moyen était de 7,6 ± 3,7 ans (3-14 ans). La majorité des enfants avait moins de 10 ans (75%). Le sex-ratio était de 2,2 (68% garçons et 32% filles). Les céphalées étaient notées chez tous les enfants, les vomissements 56,3%, et les troubles visuels 37,5% des cas. Les autres signes étaient les troubles de conscience l’amblyopie et les convulsions. Il y avait une hypertonie 38%, un syndrome pyramidal 31%, un syndrome cérébelleux 37,5% et un syndrome méningé 25% des cas. La majorité était localisée au niveau sous-tentoriel (69%) des cas, dont 31% au niveau du 4ème ventricule. Les types histologiques suspectés à la tomodensitométrie étaient l’épendymome 3 cas (18,75%), le médulloblastome 2 cas (12,5%), l’astrocytome 2 cas (12,5%), le glioblastome 2 cas (12,5%) et le craniopharyngiome 1 cas (6,25%). L’examen anatomopathologique était réalisé chez 6 patients dont 2 médulloblastomes et 1 craniopharyngiome, les résultats n’étaient pas disponibles pour les 3 autres cas. Le traitement était médico-chirurgical. La mortalité était élevée 10 cas (62,5%).
Conclusion : Les tumeurs cérébrales de l’enfant sont rares. La localisation sous-tentorielle est la plus fréquente. Les signes sont dominés par le syndrome d’hypertension intracrânienne. La mortalité reste cependant élevée.
Background: Brain tumors constitute a set of intracranial neoplasms. Their symptomatology associates an Intracranial Hypertension Syndrome (ICHS) and focal neurological syndrome as a motor deficit. The CT scans and magnetic resonance imaging are of great diagnostic value. The objective of our work was to study the epidemiological, clinical and diagnosis features of brain tumors in children hospitalized at the National Hospital for Children Albert Royer (CHNEAR) in Dakar.
Materials and methods: This was a retrospective study of brain tumors (January 1st, 2005 to December 31st, 2014). Diagnosis was performed by CT. Data were collected from patients and hospital records.
Results: Of the 30 patients with brain tumors, only 16 cases were included in our study. The incidence was 0.07%, the age ranged from 3 years to 14 years with an average age of 7.6 ± 3.7 years. The majority of children were under 10 years of age in 75% of the cases. The sex ratio is 2.2 (69% boys and 31% girls). All children were presented with headache, vomiting in 56.3% of cases and visual impairment in 37.5% of cases. Other clinical signs are disorders of alertness, amblyopia and convulsive seizures. Neurological examination found papillary edema in 4 patients, hypertonia in 38%, pyramidal syndrome in 31% of cases, cerebellar syndrome in 37.5% of cases, and meningeal syndrome in 25% of cases. There were 69% of infratentorial location and 31% in the fourth ventricle. The histological types suspected on cerebral tomodensitometry are: ependymoma 3 cases (18.75% of cases), medulloblastoma 2 cases (12.5% of cases), astrocytoma 2 cases (12.5% of cases), Glioblastoma 2 cases (12.5% of cases) and craniopharyngioma 1 cases (6.25% of cases). Six patients had histopathological examination including 2 medulloblastomas and 1 craniopharyngioma, the results were not available for the other three cases. Some patients required preoperative medical treatment (analgesic and neuro-resuscitation). The surgical treatment consisted of a ventriculo-peritoneal derivation in 4 cases, a tumoral excision in 2 cases. Mortality was high in 10 cases (62.5%).
Conclusion: Brain tumors in childhood are rare. The symptomatology associates the ICHS and the signs of neurological location. The infratentorial location is predominant. Computed tomography is a great contribution to confirm the diagnosis and topography. It also makes it possible to suspect the histological type. Mortality however remains high in our country.
