Résumé

Objectif : Le but de ce travail était d’évaluer les résultats de la radiothérapie dans le traitement du rétinoblastome.
Patients et méthodes : Il s’agit d’une étude rétrospective entre le 1er juin 2015 et le 31 juin 2017, de dix enfants présentant un rétinoblastome et traités par irradiation externe au service de radiothérapie de l’hôpital du Mali selon le protocole du Groupe Franco-Africain d’Oncologie Pédiatrique (GFAOP).
Résultats : L’âge moyen était de 51,6 mois (extrêmes : 36-60 mois). Il s’agissait de sept filles et trois garçons. Aucun antécédent familial de rétinoblastome n’a été trouvé. Le rétinoblastome était unilatéral dans neuf cas et bilatéral dans un cas. Selon la classification de Rees-Ellsworth les stades étaient les suivants : groupe IV (n = 2), et groupe V (n = 5). Tous les patients ont reçu deux cures de chimiothérapie néo-adjuvante puis énucléation. Après énucléation, ils ont reçu une radiothérapie orbitaire à la dose de 45Gy en 1.8Gy/fraction en 25 fractions. La chimiothérapie adjuvante a inclus deux cures de CE et trois cures de Carboplatine-Vincristine (CO). Après un suivi médian de 2.5 ans, la survie globale est de 100%.
Conclusion : La radiothérapie améliore le taux de contrôle local des rétinoblastomes avancés.

Summary
Radiotherapy in the treatment of retinoblastoma: About 10 cases

Purpose: The aim of this study was to evaluate the results of radiotherapy in the treatment of retinoblastoma (RB) in Mali.
Patients and methods: It was a retrospective study conducted from June 1st, 2015 to June 31st, 2017, of ten children with RB were treated with radiation therapy at National hospital of Mali, Radiation therapy service, according to the RB treatment guidelines of French African Children Oncology Group (FACOG).
Results: The median age was 51 months (36-60 months). There were seven girls and three boys. We did not find family history of RB. Unilateral RB represented nine cases. Bilateral RB represented one case. According to Reese-Ellsworth classification the staging was as follow: group IV (n = 2) and group V (n = 7). All patients received two courses of neo-adjuvant chemotherapy with of Carboplatin-Etoposide (CE) followed by enucleation. After enucleation, they received systematically early radiotherapy (with total dose of 45Gy; 1.8Gy/fraction). Adjuvant chemotherapy included two courses of CE chemotherapy and three courses of carboplatin -vincristine (CO) chemotherapy. The overall survival rate of the whole treatment strategy was 100%, with one a median follow- up of 2.5 years.
Conclusion: The use of radiation therapy can improve the local control of advanced RB.
