But : Etudier les aspects évolutifs de l’insuffisance cardiaque à travers le dosage du Pro BNP dans le service de cardiologie du Centre Hospitalier Universitaire Yalgado Ouedraogo (CHU-YO).
Patients et méthode : Il s’est agi d’une étude de cohorte prospective de patients vus en consultation externe ou en hospitalisation dans le service de cardiologie du CHU-YO sur une période de 10 mois allant du 1er janvier 2015 au 31 octobre 2015. Les patients ont été vus à leur inclusion, à 1 mois, à 3 mois et éventuellement à 6 mois. Les données collectées étaient cliniques, paracliniques et évolutives avec le dosage du NT-ProBNP.
Résultats : Au total, 54 patients ont été colligés ; l’âge moyen était de 56,89 ans avec un sex-ratio de 2,6. A l’inclusion, 70,37% de ces patients étaient en stade III ou IV de la NYHA. Les principales étiologies de l’IC étaient la cardiopathie hypertensive, la cardiomyopathie dilatée d’allure primitive et la cardiopathie ischémique ; la FEVG moyenne de 35,94 ; Le NT-ProBNP moyen de 4458,93 pg/ml avec une bonne performance diagnostique du test était corrélée au stade de la NYHA, à la FEVG et aux PRVG. La majorité de nos patients a bénéficié d’un traitement optimal et au cours du suivi l’évolution était marquée par une diminution des taux de NT-ProBNP, de la FEVG et des signes d’IC avec un faible taux de réhospitalisation à 3,70%.
Conclusion : Le NT-ProBNP représente l’avantage d’un biomarqueur fiable et d’une bonne sensibilité dans le cadre de l’exploration, d’évaluation et de suivi de l’IC.
Aim: To study the progressive aspects of heart failure through Pro BNP test in the department of cardiology of Yalgado Ouedraogo University Teaching Hospital (YO UTH).
Patients and method: It was a prospective cohort study of patients seen in outpatient consulting room or in admission in the department of cardiology of YOUH. The study was performed over a ten-month period, from January 1st, 2015 to October 31st, 2015. Patients were seen on enrolment, then reviewed after one (1) month, three (3) months and possibly six (6) months. Clinical, paraclinical and disease progression data, with NT-Pro BNP test were collected.
Results: A total of 54 patients were enrolled; mean age was 56.89 years and sex-ratio was 2.6. On inclusion, 70.37% of patients were in NYHA class III or IV. The main causes of heart failure were hypertensive heart disease, primary congestive cardiomyopathy and ischemic heart disease. Mean Left Ventricular Ejection Fraction (LVEF) was 35.94%; mean NT-Pro BNP was 4458,93 pg/ml and was correlated with a good diagnostic performance to the NYHA class, the LVEF and the left ventricular filling pressure. Majority of our patients had optimal treatment; during their follow up, NT-Pro BNP rates got decreased along with congestive signs of heart failure. LVEF got increased and in hospital readmission rates were low (3.70%).
Conclusion: NT-Pro BNP has the benefit of a reliable biomarker with a good sensitivity, in the screening, the assessment and the follow up of heart failure.
