Résumé

Introduction : Le carcinome papillaire intra-kystique est un type histologique rare de cancer du sein. La chirurgie est la pierre angulaire du traitement. Nous rapportons un cas de carcinome papillaire intra-kystique du sein associé à des foyers de carcinome non spécifique infiltrant chez une patiente âgée de 56 ans.
Observation médicale : Il s’agissait d’une patiente âgée de 56 ans, ménopausée, Elle a consulté à la clinique Universitaire de Chirurgie Viscérale du CNHU Hubert Koutoukou Maga en juillet 2020 pour un nodule du sein découvert à l'autopalpation 2 ans plus tôt. Il existait au niveau des quadrants supérieurs du sein gauche, deux masses estimées respectivement à 5 cm et 15 cm d'allure clinique bénigne. La mammographie, l'échographie et la cytologie étaient en faveur de lésions bénignes. Elle a bénéficié dans un premier temps d’une exérèse large. L’examen anatomo-pathologique de la pièce opératoire révélait un carcinome papillaire intra-kystique associé à des foyers de carcinome mammaire infiltrant de type non spécifique. Il a ensuite été réalisé une mastectomie radicale gauche avec curage axillaire homolatéral. Cinq mois après la chirurgie, l’examen de la patiente n’objective aucun signe de récidive.
Conclusion : Le carcinome mammaire intra-kystique est une forme rare de cancer du sein, de bon pronostic. Il peut être associé à un carcinome non-spécifique invasif. La chirurgie est la base du traitement.

Intracystic papillary carcinoma of the breast associated with foci of non-specific invasive carcinoma

Introduction: Intracystic papillary carcinoma is a rare histological type of breast cancer. Surgery is the cornerstone of treatment. We report a case of intracystic papillary carcinoma of the breast associated with infiltrating carcinoma in 56-year-old women.
Case report: A 56-year-old menopausal women presented at the Visceral Surgery Department of the National University Teaching Hospital of Cotonou (NUTH Hubert Koutoukou Maga) in July 2020 for a breast mass discovered during self-examination, 2 years earlier. There were two masses in the upper quadrants of the left breast, estimated at 5 cm and 15 cm respectively, of benign clinical appearance. Mammography, ultrasound, and cytology concluded to a benign tumor. She underwent a wide resection in the first instance. Pathology concluded to an intracystic papillary adenocarcinoma associated with three foci of infiltrating breast carcinoma. A left radical mastectomy with homolateral axillary lymph node dissection was then performed. Five months after surgery, she has no clinical sign of recurrence.
Conclusion: Intracystic breast carcinoma represents a rare entity of breast cancers with a good prognosis. It can be associated with an invasive carcinoma. Surgery is the cornerstone of the treatment.
