Les auteurs rapportent deux cas de cancer de l’ampoule de Vater. Il s’est agi de deux femmes âgées de 31 et 70 ans. Le tableau clinique a été celui de douleur abdominale, ictère, gros foie, grosse vésicule, altération de l’état général. La biologie a été marquée par une anémie microcytaire et hypochrome, une cholestase, une cytolyse hépatique dans les deux cas, mais plus importante chez la patiente âgée de 31 ans. Chez la même patiente l’AgHBs est positif ainsi que l’anticorps anti-HBc de type IgG. L’échographie abdominale a confirmé le gros foie, la grosse vésicule et a révélé une dilatation des voies biliaires intra et extra-hépatiques. La lithiase vésiculaire et de la voie biliaire principale a été retrouvée dans un cas. L’endoscopie digestive haute a retrouvé des lésions ulcéro-bourgeonnantes de la papille duodénale dont l’analyse histologique a conclu à un adénocarcinome Lieberkühnien.
Le cancer de l’ampoule de Vater a été associé à un adénocarcinome Lieberkühnien du rectum chez la patiente âgée de 31 ans. Les deux malades n’ont pu bénéficier d’un traitement chirurgical à cause de l’altération profonde de l’état général ayant conduit au décès dans les deux cas. Le cancer de l’ampoule de Vater est rare. Son expression clinique est non spécifique et tardive. Le pronostic est globalement mauvais à cause du retard du diagnostic.
The authors report two cases of cancer of the ampulla of Vater. It was about two women aged of 31 and 70 years. The clinical picture was that of abdominal pain, jaundice, big liver, big bladder, altered general status. Biology has been marked by a microcytic and hypochromic anemia, cholestasis, hepatic cytolysis in both cases, but more important in the patient 31 year-old patient. In the same patient, the AgHBs is positive as well as the antiHBc of IgG type antibody. Echo abdominal scan confirmed the large liver, the large gallbladder and revealed a dilation of the biliary intra and extrahepatic ways. Vesicular and main biliary way lithiasis was found in a case. High digestive endoscopy found ulcero fungating lesions of the duodenal papilla whose histological analysis concluded in a Lieberkühnien adenocarcinoma.
The ampulla of Vater cancer has been associated with a Lieberkühnien adenocarcinoma of the rectum in the 31 year-old patient. The two patients could not benefit from surgical treatment because of the profound alteration of the general state that led to the death in both cases. Cancer of the ampulla of Vater is rare. Its clinical expression is no specific and late. The prognosis is generally bad because of the delay of the diagnosis.
