Introduction : La prévalence des événements cardio-vasculaires est nettement plus élevée chez les patients hémodialysés que dans la population générale. Le but de notre étude était de déterminer les anomalies électrocardiographiques et échocardiographiques chez des patients hémodialysés chroniques à Dakar.
Patients et méthodes : Il s’agissait d’une étude transversale, rétrospective de type descriptive menée chez les patients hémodialysés chroniques aux unités d’hémodialyse de deux centres hospitalo-universitaires de Dakar. Ont été inclus les patients hémodialysés chroniques ayant bénéficié d’une automesure tensionnelle à domicile, d’une échocardiographie et/ou d’un électrocardiogramme à 12 dérivations dans la période d’étude.
Résultats : Quarante-six patients avaient été inclus. L’âge moyen était de 44,76 ± 14,51 ans avec un sex-ratio de 1,6. La durée moyenne en hémodialyse était de 59,61 ± 36,69 mois. L’HTA était présente chez 71,7% des patients. Les anomalies électrocardiographiques étaient dominées par les troubles de la conduction (36,6%), l’ischémie sous épicardique (46,3%), l’hypertrophie ventriculaire gauche (36,6%) et l’hypertrophie auriculaire gauche (63,4%). Les principales anomalies échocardiographiques étaient représentées par l’hypertrophie excentrique du ventricule gauche (25%), l’hypertrophie concentrique du ventricule gauche (25%), la dilatation de l’oreillette gauche (40%) et l’hypertension artérielle pulmonaire (20%).
Conclusion : Les anomalies électrocardiographiques et échocardiographiques sont très fréquentes chez les patients hémodialysés à Dakar. Dans notre étude, 90,2% des patients avaient un électrocardiogramme pathologique.
Introduction: Prevalence of cardiovascular events is significantly higher in hemodialysis patients than in the general population. The aim of our study was to determine electrocardiographic and echocardiographic abnormalities in chronic hemodialysis patients in Dakar.
Patients and methods: This was a cross-sectional, retrospective and descriptive study conducted in chronic hemodialysis patients at two hemodialysis units of Dakar university hospitals. All chronic hemodialysis patients with home blood pressure, echocardiography and / or 12-lead electrocardiogram was included.
Results: Forty-six patients were included. Mean age was 44.76 ± 14.51 years with a sex ratio of 1.6. Mean duration of hemodialysis was 59.61 ± 36.69 months. High blood pressure was present in 71.7% of patients. Electrocardiographic abnormalities were dominated by conduction disturbances (36.6%), epicardial ischemia (46.3%), left ventricular hypertrophy (36.6%) and left atrial hypertrophy (63.4%). Echocardiographic abnormalities were eccentric hypertrophy of the left ventricle (25%), concentric hypertrophy of the left ventricle (25%), dilation of the left atrium (40%) and pulmonary arterial hypertension (20%).
Conclusion: Electrocardiographic and echocardiographic abnormalities are very common in hemodialysis patients in Dakar. In our study, 90.2% of patients had a pathological electrocardiogram.
