L’hématome spontané de la cuisse est une affection très rare et habituellement observée au niveau du muscle psoas au cours des traitements anti-coagulants ou des troubles de l'hémostase tels que l'hémophilie. Le but de notre travail était de décrire les aspects diagnostiques, et évolutifs d’un hématome de la cuisse droite chez un adolescent de 13 ans révélant une leucémie myéloïde chronique. L’échographie et la tomodensitométrie de la tuméfaction du tiers moyen de la cuisse droite ont confirmé l’hématome. L’hémogramme a orienté l’étiologie de l’hématome (hyperleucocytose avec myélémie équilibrée, thrombocytose). L’hybridation in situ fluorescente a confirmé le diagnostic de leucémie myéloïde chronique (gène BCR-ABL). Le traitement par imatinib a permis d’obtenir la disparition de l’hématome, la réponse hématologique complète à 8 mois de traitement et une réponse moléculaire majeure à 44 mois de traitement.
Spontaneous hematoma of the thigh is a very rare affection usually seen in the psoas muscle during anticoagulant therapy or in hemostasis disorders such as hemophilia. The aim of our work was to describe the diagnostic and evolutionary aspects of a hematoma of the right thigh in a 13-year-old adolescent with chronic myeloid leukemia. Ultrasound and CT scan of the swelling of the middle third of the right thigh confirmed the hematoma. The blood count guided the etiology of the hematoma
(hyperleukocytosis with balanced myelemia, thrombocytosis). Fluorescent in situ hybridization confirmed the diagnosis of chronic myeloid leukemia (BCR-ABL gene). Treatment with imatinib resulted in disappearance of the hematoma, complete hematologic response at 8 months of treatment and a major molecular response at 44 months of treatment.
