L’arthropathie destructrice est une des complications de la goutte chronique à l’origine de déformations articulaires. Nous rapportons un cas clinique de goutte chronique tophacée atypique déformante et invalidante chez un patient de 69 ans qui a été admis en hospitalisation pour une polyarthrite chronique évoluant par poussée-rémission depuis 25 ans intéressant les grosses et petites articulations avec participation axiale du rachis cervical ; laquelle est devenue invalidante avec une perte de l’autonomie faite d’une impossibilité à la marche, ankylose des mains, coudes, genoux associée à des paquets de nodules sous-cutanés disséminés sur tout le corps. Le patient est actuellement confiné dans un fauteuil roulant. Le bilan biologique a noté un syndrome inflammatoire, une hyperuricémie à 82,5 mg/L ; le bilan immunologique est revenu négatif. La biopsie des nodules sous cutanés a conclu à un tophus goutteux et à la radiographie, on note des images radiologiques destructrices faites de géodes juxta-articulaires associées à des encoches latérales et de déminéralisation osseuse avec pincement des articulations touchées. Le diagnostic retenu était celui d’une goutte chronique tophacée avec arthropathie uratique destructrice. Le patient a alors été traité par colchicine ; allopurinol associé à des mesures hygiéno-diététiques. La particularité de ce cas clinique est que les destructions et déformations s’étendent aux grosses articulations (épaules, coudes, genoux) et au rachis cervical avec les tophi sous-cutanés qui sont disséminés sur tout le corps (aux quatre membres et au tronc).
Destructive arthropathy is one of the complications of chronic gout, source of joint deformations. We are reporting a clinical case of atypical chronic tophaceous deforming and disabling gout in a 69 years-old patient who has been admitted in hospitalization for chronic polyarthritis characterized by outbreak and remission periods since 25 years affecting small and large joints involving the cervical spine’s axis. That became disabling with a loss of autonomy made of being unable to walk, ankylosis of hands,
elbows, legs associated to complex and subcutaneous nodules scattered on the whole body. The patient is currently confined to a wheelchair. The biological tests revealed an inflammatory syndrome, a hyper-uricemia at 82.5 mg/L. The immunological tests were negative. The biopsy of the subcutaneous nodules found a gouty tophus as conclusion. The radiography noted erosive images consisting of juxta-articular geodes associated to lateral notches, bone demineralization and a narrowing of the affected joints. The diagnosis retained was chronic tophaceous gout with an erosive uratic arthropathy. The patient has been treated using Colchicine, Allopurinol combined to diet and lifestyle measures. The particularity of this clinical case is the large joints destructions, cervical spine and large joints (shoulders, elbows, legs) deformations with subcutaneous tophi scattered on the whole body (the 4 limbs and the trunk).
