Résumé

Objectif : Rapporter un cas de corps étranger à point de départ oropharyngé avec migration cervicale observé au CHU de Cocody.

Cas clinique : Nous rapportons un cas de corps étranger oropharyngé méconnu pendant 3 mois avec migration cervicale, chez une patiente de 6 ans. Il a été révélé au décours d’une cervicotomie exploratrice devant un tableau d’hémorragie cervicale survenu lors d’un pansement. Outre la découverte du corps étranger (arête de poisson), il existait une ouverture de la loge sub-mandibulaire droite et une lésion de l’artère faciale. L’évolution à court terme était marquée par une suppuration cervicale. Un traitement médical, à cet effet, fait d’instillation d’Ofloxacine localement a permis d’obtenir des suites opératoires favorables.

Conclusion : Ce cas clinique illustre les complications possibles d’une arête de poisson méconnue, la nécessité d’un examen clinique minutieux et la prise en charge en milieu spécialisé.

Summary
Unusual migration of an oropharyngeal foreign body

Objective: To report a case of foreign body starting point with oropharyngeal cervical migration observed at the University Hospital of Cocody.

Case report: We report a case of 3 months unknown oropharyngeal foreign body with cervical migration, in a 6- year-old female patient. It was revealed the waning of explorer cervicotomy before a cervical bleeding occurred during a table dressing. Besides the discovery of the foreign body (fish bone), there was an opening of the lodge right submandibular and the facial artery lesion. The short-term evolution was marked by a cervical suppuration. Medical treatment, for this purpose, made by locally ofloxacin instillation Ofloxacin yielded favorable postoperative.

Conclusion: This case illustrates the complications of an unknown fish bone, the need for careful clinical examination and care in specialized areas.
