Résumé

Introduction : Les corps étrangers du tractus aérodigestif sont fréquents en pratique ORL et constituent des urgences diagnostiques et thérapeutiques. Cependant, ceux du nasopharynx sont rarement décrits, en particulier chez l’adulte.
Nous rapportons un cas de corps étranger (prothèse dentaire) enclavé dans le nasopharynx d’un adulte jeune au CHU Souro Sanou. Nous présentons les particularités cliniques et paracliniques et faisons une revue de la littérature.

Observation : Une femme de 40 ans, sans antécédents pathologiques connus, porteur d’une prothèse dentaire depuis environ 5 ans, avait été reçue pour des douleurs cervicales antérieures, des épistaxis et des saignements de l’oropharynx, après avoir avalé la prothèse accidentellement.
Devant un tableau d’hypersialorrhée, de respiration buccale, de rhinolalie fermée et de saignements par la bouche et les narines, un corps étranger du nasopharynx avait été suspecté. Il avait été confirmé par une radiographie standard du cavum de profil qui montrait une opacité verticale hétérogène enclavée, avec des extrémités en forme de crochets (opacité métallique), s’étendant de l’arrière des choanes à une ligne horizontale passant par le bord inférieur de l’atlas. Les suites opératoires de l’endoscopie d’extraction sous anesthésie générale avec intubation trachéale avaient été favorables.

Conclusion : Une démarche clinique rigoureuse, l’apport de l’imagerie et de l’endoscopie, permettent le diagnostic et la prise en charge précoce des corps étrangers du nasopharynx.

Summary
Unusual localization in the cavum of a young adult of a foreign body: about a case and review of literature

Introduction: Foreign bodies of the aerodigestive tract are common in ENT practice and constitute diagnostic and therapeutic emergencies. However, those of the nasopharynx are rarely described, especially in adults.

Patient and method: We report a case of foreign body (dental prosthesis) embedded in the nasopharynx of a young adult at Souro Sanou Teaching Hospital. We present the clinical and
paraclinical peculiarities and make a review of the literature.

Observation: A 40-year-old woman with no history of a pathological condition who had had a dental prosthesis for about five years, was admitted for the anterior neck pain, epistaxis, and bleeding from the oropharynx, after accidentally swallowing the prosthesis. In front of hypersialorrhoea, mouth breathing, closed rhinolalia and bleeding from the mouth and nostrils, a foreign body of the nasopharynx had been suspected. It had been confirmed by a standard radiograph in profile of the cavum, that showed a heterogeneous and vertical opacity enclosed, with hook-shaped ends (metallic opacity), extending from the back of the choanes to a horizontal line passing through the lower edge of the atlas. The operative follow-up of endoscopy extraction under general anesthesia and tracheal intubation was favorable.

Conclusion: A rigorous clinical approach, the contribution of imaging and endoscopy, allow the diagnosis and early management of foreign bodies of the nasopharynx.
