Résumé

Le myxome est une variété rare de tumeurs bénignes des maxillaires, dont l’origine serait le mésenchyme embryonnaire du follicule dentaire.
Les manifestations cliniques et radiologiques sont variables et non-spécifiques et peuvent à prêter confusion avec d’autres lésions.
Nous décrivons un cas de myxome odontogène du maxillaire très envahissant, observé chez une femme de 51 ans. Cliniquement, la patiente présentait une tuméfaction maxillaire gauche, indolore et ferme à la palpation. Une TDM du massif facial montrait un processus expansif ostéolytique soufflant le maxillaire.
Le traitement a consisté en une exérèse large de la tumeur par voie endobuccale, la tumeur s’individualise bien du plan osseux. L’examen anatomopathologique de la pièce d’exérèse a posé le diagnostic de myxome odontogène. L’évolution à court terme n’a pas montré de récidive locale clinique et radiologique.
Le diagnostic s’est basé sur des arguments cliniques, radiologiques et histologiques. Cette tumeur est localement agressive, pouvant entraîner d’importantes déformations faciales et des perturbations dentaires. Le caractère récidivant du myxome impose un traitement radical large dépassant les limites de la lésion. La perte de substance importante impose une réparation chirurgicale ou prothétique.

Abstract
Odontogenic myxoma of the maxilla: report of a case

Myxoma is a rare variety of benign tumors of the jaw, whose origin is embryonic mesenchymal dental follicle.
Clinical and radiological manifestations are variable and non-specific and can lend confusion with other lesions.
We describe a case of very invasive odontogenic myxoma , observed in a woman of 51 years.
Clinically, the patient had left maxillary swelling, painless, firm to the touch. CT facial bones showed an expansive osteolytic process blowing the jaw.
The treatment consisted of a wide excision of the tumor by endobuccal way, the tumor is individualized well from the bone. Pathological examination of the surgical specimen was diagnosed odontogenic myxoma. The short-term trend has shown no clinical and radiological local recurrence.
The diagnosis is based on clinical, radiological and histological. This tumor is locally aggressive, which can lead to significant facial deformities and dental disturbances. The recurrent nature of myxoma imposes a broad radical treatment beyond the limits of the lesion. The loss of important substance required surgical repair or prosthetic.
