Résumé

Afin de poursuivre le parallélisme entre déterminants génétiques HLA, polyarthrite rhumatoïde et affections parodontales, le but de ce travail était d’examiner si le postulat des épitopes HLADRB107, HLADRB13, HLADRB11 et HLA-DERAA réputés "protecteurs" pour la polyarthrite rhumatoïde l’était également pour les affections parodontales sévères.
Matériels et méthodes : Dans le cadre d’une étude cas-témoins, (base de données de 1999 recueillies par notre équipe), l’étude a été menée sur une population d’origines ethno-géographiques diverses. L’échantillon se compose de 48 patients (cas) âgés de 20 à 48 ans atteints de parodontites sévères et/ou à progression rapide et d’un panel de 55 patients donneurs du Centre Régional de Transfusion sanguines reconnus comme témoins.
Résultats : On note un effet protecteur statistiquement significatif de HLA B107 sur la parodontite sévère après ajustement sur l’âge et le niveau de plaque OR = 0,271 [IC 95% 0,089-0,824] ; p = 0,009. L’effet protecteur HLADRB107/parodontites sévères se maintient après stratification selon l’origine ethno géographique (χ² de Mantel-Haentzel = 4,093 ; risque de première espèce p = 0,043). L’OR correspondant à la liaison HLADRB107/ parodontites sévères étant égal à 0,272 [IC à 95% 0,077-0,960]. Cet effet protecteur n’est pas révélé lorsqu’on applique les mêmes modèles HLAB107 et parodontites dites à progression rapides. De même, aucune influence (effet protecteur) n’a été révélée entre les épitopes HLA-DERAA, HLADR13, HLADR11 et les affections parodontales ; les Odds Ratio étant non-significatifs.
Conclusion : Cette étude a mis en évidence un effet protecteur du HLADRB107 sur le développement des maladies parodontales sévères soulignant le parallélisme avec les interactions physiopathologiques, HLA et polyarthrite rhumatoïde.

Abstract
Influence of HLAB107 on severe periodontitis: A protective effect

To continue the parallelism between HLA, rheumatoid arthritis and periodontal disease, the aim of this work was to examine whether the postulate of the epitops HLADRB107, HLADRB13, HLADRB11 and HLA-DERAA, which are known to be “protective" for rheumatoid arthritis remains concerning periodontal affections.
Materials and methods: In a case-control study (1999 database collected by our team), the study was carried out on a population of various ethno-geographical origins. “Cases” or patients (48 subjects) had to be aged between 20-48 years and presented severe periodontitis or rapidly progressive periodontitis. A panel of 55 blood donor’s subject from the Regional Blood Transfusion Center was recognized as “controls”.
Results: There is a statistically significant protective effect of HLA B1 07 on severe periodontitis after adjustment to age and level of dental plaque; OR = 0.271 [95% CI 0.089-0.824]; p = 0.009. The protective effect HLADRB107/severe periodontitis still effective after stratification according to the ethno-geographical origin (χ² of Mantel-Haentzel = 4.093, risk of first species p = 0.043). The OR corresponding to the link HLADRB107/severe periodontitis is equal to 0.272 [95% CI 0.077-0.960]. This protective effect is not revealed if the same model is applied to rapidly progressive periodontitis and HLAB107. Similarly, they are no protective effect between the epitops HLA-DERAA, HLADR13, HLADR11 and periodontal affections: the statistical significance being not established.
Conclusion: This study demonstrates a protective effect of HLADRB107 on the development of severe periodontal disease, emphasizing parallelism with HLA, rheumatoid arthritis and physiopathology interactions.
