Résumé

Objectif : Evaluer l’influence de la valeur du "cement gap" sur la précision de l’adaptation marginale des coquilles fabriquées suivant le système Lava, après scellement avec deux ciments différents.
Matériels et méthodes : 54 modèles de la même taille d’une molaire inférieure de dentoforme ont été divisés en 6 groupes pour recevoir des cores Lava. Les groupes diffèrent par la valeur de "cement gap" : 20-40-60 μm et par le ciment utilisé : RelyX Luting2 ou RelyX U200. L’adaptation marginale a été mesurée sous stéréoscope avant et après scellement. Une coupe sagittale a été effectuée pour pouvoir mesurer l’adaptation interne.
Résultats : Il y avait une différence significative entre les résultats obtenus pour les 2 types de ciments. Pour RelyX U200, pas de différence significative avant et après scellement. Avec le RelyX Luting2, il y a une différence significative entre les 3 groupes de "cement gap". Pour le groupe de "cement gap" 60 μm, la différence entre les deux types de ciments n’était pas significative. L’adaptation interne était différente suivant le type de ciment, mais ne diffère pas en fonction de la valeur de cement gap.
Conclusion : les résultats de cette étude indiquent qu’il faut penser à adapter le ‘cement gap’ suivant le type de ciment utilisé. Avec un ciment assez fluide comme le RelyX U200, cette variable n’affecte pas l’adaptation marginale. Mais dans le cas d’un ciment comme le RelyX Luting2, il est possible d’améliorer cette adaptation en passant d’une valeur de "cement gap" de 20 μm à 60 μm, sans affecter l’adaptation interne.
Signification clinique : lors de la préparation des cores en zircone, la valeur de "cement gap" doit prendre en considération le type de ciment à utiliser. L’augmentation de la valeur de "cement gap" de 20 à 60 μm améliore l’adaptation marginale des couronnes scellées avec RelyX Luting2.

Abstract
In vitro evaluation of the adaptation of zirconia crowns and its variation after cementation

Aim: To evaluate the effect of the “cement gap” on the precision of fit of CAD/CAM zirconia cores after cementation with two different cements.
Materials and methods: An inferior molar of a dentoform was prepared and duplicated into 54 stone dies in order to get Lava cores. The specimens were divided into six groups that differ in the value of “cement gap”: 20, 40 and 60 μm; and the cement used for cementation: RelyX Luting2 or RelyX U200. Marginal gap was measured before and after cementation. An axial section was performed to measure the internal gap.
Results: A significant difference was found between the two cements. With RelyX U200, no difference was found before and after cementation. With RelyX Luting2, the difference was significant between the three groups of “cement gap”. The difference between the two types of cement was not significant for the group of “cement gap” = 60μm but was significant for the groups of 20μm and 40μm. Internal gap differed according to the type of cement, though there was no difference between groups cemented with the same type of cement.
Conclusion: The value of “cement gap” should be chosen depending on the cement that will be used. When a thicker cement like RelyX Luting2 is used; better marginal adaptation can be obtained when the “cement gap” is increased to 60 μm, without interfering with internal gap.
Clinical significance: While preparing the design for zirconia crowns, the value of “cement gap” should take into considerations the type of cement that will be used for cementation. The augmentation of the cement gap from 20 to 60 μm improved the marginal adaptation of crowns cemented with Rely-x Luting2.
