Résumé

Le traitement par implants a considérablement modifié la dentisterie moderne. Pour garantir le succès de cette dernière, il est nécessaire d’élaborer un plan de traitement approprié en tenant compte à la fois de la partie chirurgicale et de la partie prothétique. Souvent, les praticiens ont tendance à élaborer un plan de traitement en négligeant les principes de base de la partie prothétique. La restauration implanto-portée d’une molaire mandibulaire a toujours été un défi en termes de forme et de fonction. En outre, les dimensions mésio-distales d’une molaire dépassent celles de la plupart des implants standard, ce qui crée la possibilité d’une surcharge fonctionnelle conduisant à une défaillance du pilier ou de l’implant. Par conséquent, les paramètres cliniques régissant le projet de restauration doivent être soigneusement évalués à la lumière de la disponibilité des implants et des composants qui offrent une myriade d’options pour les diamètres, les configurations de plate-forme et les connexions prothétiques.
L’objectif de cet article était d’examiner, à l’aide d’une étude de cas, les particularités de la gestion des implants et les exigences à respecter depuis l’étude pré-implantaire jusqu’au scellement des prothèses implanto-portées. Un cas clinique détaillé étape par étape avec ses indications a été discuté pour aider tous les praticiens des implants dentaires à élaborer un plan de traitement optimal dans chaque cas.

Guidelines ensuring longevity of implant supported crown replacing mandibular molar: a clinical report

Modern dentistry has changed tremendously with implant therapy. To guarantee the success of the latter, making a proper treatment plan considering both surgical and prosthetic part in mind is necessary. Often practitioners tend to create a treatment plan overlooking the basic principles of prosthetic part. The implant-supported restoration of a mandibular molar has historically been a challenge in terms of form and function. Furthermore, the mesio-distal dimensions of a molar exceed those of most standard implants, creating the possibility of functional overload which leads to abutment or implant failure. Therefore, the clinical parameters governing the restorative project must be carefully assessed in light of the availability of implants and components that provide a myriad of options for diameters, platform configurations and prosthetic connections.
The aim of this paper was to use a case study to examine the particularities of implant management and the requirements to be met from the pre-implant study to the cementing of implant-supported prostheses. A step-by-step detailed clinical case options with their indications has been discussed to help all dental implant practitioners to make an optimal treatment plan for each case.
