Aspects cliniques et thérapeutiques de la névralgie faciale essentielle du trijumeau. A propos de 27 cas colliges à l’hôpital général de Grand-Yoff de Dakar

La névralgie faciale essentielle (NFE) du trijumeau (V) ou «tic douloureux» de la face est une affection invalidante, qui affecte la qualité de vie du patient. L’odontologiste doit être capable de la diagnostiquer et de participer à sa bonne prise en charge thérapeutique. Le diagnostic est parfois difficile, pouvant entraîner des modalités thérapeutiques inadaptées préjudiciables pour le patient. L’objectif de notre étude était d’analyser les aspects cliniques et thérapeutiques de la NFE du V. Il s’agit d’une étude transversale descriptive qui a colligé 27 dossiers de patients souffrant de NFE du V et suivis dans le service d’odontologie de l’Hôpital Général de Grand-Yoff (HOGGY) à Dakar entre juillet 2002 et juillet 2008. Nous avons exploité les paramètres suivants : l’âge, le sexe, l’état bucco-dentaire, la durée des symptômes, les caractéristiques de la douleur, le type de traitement. La moyenne d’âge était de 53 ans, avec des extrêmes à 22 et 73 ans et un écart-type de 12,9. Sur les 27 cas, 22 patients étaient de sexe féminin. Le délai de consultation a été souvent tardif, les patients préférant le plus souvent débuter par les traitements traditionnels. La névralgie du V2 a dominé le tableau clinique, suivi de celle du V3. La douleur qui était strictement unilatérale, déclenchée par le moindre mouvement, la parole, l’alimentation ou l’affleurement de la zone gâchette.
La prescription de la carbamazépine (Tégrétol®) a été systématique, et a constituée un véritable test diagnostic, elle a permis d’obtenir une sédation immédiate des douleurs.
Devant l’échec du traitement médicamenteux, l’alcoolisation de la branche trigéminale a donné de bons résultats, en effet elle a permis d’obtenir, une nette accalmie de plus de 12 mois chez 09 patients sur les 10 qui en ont bénéficié.
L’alcoolisation du trijumeau reste un traitement efficace et constitue une alternative accessible dans nos conditions de travail en Afrique.
The trigeminal neuralgia or "painful tic" of the face is an invalidating affection, which affects the quality of life of the patient. The odontologist must be able to diagnose it and take part in his good therapeutic assumption of responsibility. The diagnosis is sometimes difficult, being able to involve unsuited therapeutic methods prejudicial for the patient.
The objective of our study was to analyse the clinical and therapeutic aspects of the trigeminal neuralgia. It was a transverse and descriptive study which 27 files of patients suffering of trigeminal neuralgia and treated in the odontology service of the General hospital of Grand-Yoff in Dakar between July 2002 and July 2008.
We exploited the following parameters: the age, the sex, the oral state, duration of the symptoms, characteristics of the pain, the type of treatment. 22 patients of the 27 cases were female. The time of consultation was often late, the patients generally preferring begin with the traditional treatments. The neuralgia of V2 dominated the clinical picture, followed that neuralgia of V3. The pain was strictly unilateral, it was started by movement, speaking, eating or touching the trigger zone. The prescription of carbamazepine (Tegretol®) was systematic, and constituted a true test diagnosis, it made possible to obtain an immediate sedation of the pains. In front of the medicamentous treatment failure, the alcoholization of the trigeminal branch has given good performances, indeed it made possible to obtain, a clear lull of more than 12 months for 9 patients out of the 10 who profited from it. The alcoholization of the trigeminal nerve remains an effective cure and constitutes an accessible alternative under our work conditions in Africa.

Attitudes et perception des dentistes nigérians concernant le concept d’arcade dentaire raccourcie (ADR)

Objectif : Le traitement par raccourcissement de l’arcade dentaire est une stratégie pour réduire un traitement complexe de restauration des molaires. En offrant une dentition partielle aux patients, cette option thérapeutique assure une fonction buccale, améliore l’hygiène orale et le confort et réduit les coûts de traitement. Le concept d’arcade dentaire raccourcie apporte un avantage sans compromettre la prise en charge du patient. Malgré quelques études encourageantes, peu d’études sont consacrées au traitement par raccourcissement de l’arcade dentaire. La présente étude se propose d’étudier l’attitude et la perception des dentistes nigérians face à ce concept.
Matériels et méthodes : Une étude transversale était menée par auto-questionnaire structuré. Le questionnaire utilisé par Sarita et al. était modifié et utilisé comme instrument de recueil de données. Les questions portaient sur les variables socio-démographiques et l’attitude et la perception face au traitement par raccourcissement de l’arcade dentaire. Les questionnaires étaient distribués aux dentistes généralistes et aux internes résidents dentistes en formation dans les institutions nigérianes.
Résultat : L’expérience clinique de la plupart des participants était de £ 10 ans. Ils exerçaient en majorité dans le secteur public. Quelques uns ont indiqué que le concept était bon pour les pays en voie de développement comme le Nigéria. La majorité a indiqué que les patients à dentition réduite n’étaient pas moins sains sur le plan nutritionnel et que les fonctions orales et la santé maxillo-faciale de ces patients étaient acceptables.
Conclusion : Il apparait que l’attitude et la perception des dentistes Nigérians concernant le concept de l’arcade dentaire réduite ne sont pas significatives à l’heure actuelle.
Objective: The shortened dental arch therapy (SDAT) is a strategy to reduce complex restorative treatment in the molar area. By offering the partially dentate patients, a treatment option that ensures oral functionality, improved oral hygiene, comfort and possibly reduced costs, the SDA treatment approach appears to provide an advantage without compromising patient care. Despite these encouraging reports, it has been stated that there are few papers in the literature addressing clinical attitudes to SDAT. The current study is, therefore, designed to determine the attitude and perception of dentist in Nigeria about the concept.
Materials and methods: A cross-sectional study was conducted using a self-administered struc-tured questionnaire. The questionnaire employed by Sarita et al. was modified and employed as the instrument for data collection. The respondents were requested to provide information about their socio-demographic variables and their attitudes and perception of SDAT. The questionnaires were randomly distributed among general dental practitioners and residents (postgraduate doctors) in dental training institutions in Nigeria.
Result: The clinical experience of most of the respondents was £ 10 years. Most of the respondents were working in government institutions. Few of the respondents (36.1%) indicated that the concept was good for developing country like Nigeria and only few of the respondents (38.0%) agreed with the problem-oriented approach. Most respondents indicated that patients with shortened dental arches are not nutritionally less healthy and that, the oral functions and TMJ health of these patients is acceptable.
Conclusion: It appears the attitude and perception of dentists in Nigeria as it relates to shortened dental arch concept is not impressive at the moment.

Etude clinique comparative à 5 ans d’une céramique et d’un Ceromer utilisés pour la confection d’inlays esthétiques

Trente-six inlays également répartis entre céramique et composite de laboratoire ont été collés, en paire, sur le même type de dent chez quinze patients. Ils ont été suivis pendant 60 mois en moyenne.
Les résultats ont montré que les 2 systèmes sont acceptables cliniquement pour la restauration des dents postérieures. Cependant, pour le critère «adaptation de la teinte» les inlays en céramique montrent des résultats significativement supérieurs.
Thirty-six inlays, equally divided between a vitro ceramic and a composite resin, were bonded in pair, on the same type of teeth in fifteen patients. They were followed for a mean period of 60 months.
The results showed that both materials were clinically acceptable for restoring posterior teeth, even though ceramic yielded better results concerning colour match.

Effect of different modes of light curing and resin composites on microleakage of Class II restorations - Part II

Introduction : Pour réduire la rétraction de polymérisation des résines composites et après l’introduction de la « polymérisation lente et progressive » par GORACCI et al. en 1992, plusieurs appareils à photopolymériser ont présenté la polymérisation «soft-start», en plus du mode de polymérisation classique à intensité lumineuse maximale. Cette étude a testé si cette polymérisation lente et progressive pourra réduire le décollage marginal en diminuant le stress interne des composites, sachant que les études qui ont comparé ce mode de polymérisation sont rares.
Matériel et Méthodes : Cent vingt cavités classe II ont été préparées au niveau des faces mésiales et distales de prémolaires humaines extraites. Ces dents ont été réparties aléatoirement en 4 groupes (n = 15). Au niveau de chaque dent, les cavités mésiales ont été restaurées avec le composite micro-hybride Filtek Z250 (3M ESPE) et les cavités distales ont été restaurées avec le composite nano-chargé Filtek Suprême (3M ESPE). Dans cette étude, l’appareil à photopolymériser utilisé était un appareil halogène conventionnel, le QHL 75 (Dentsply), modifié en ajoutant un dispositif de réglage électronique, contrôlé par un logiciel spécial qui nous permet d’avoir 4 modes de polymérisation différents comme suit :
Les dents ont été stockées dans de l’eau à 37°C pendant 48 heures, polies, puis soumises à un thermocyclage (3000 cycles entre 5°C et 55°C). Toutes les dents ont été immergées dans une solution de bleu de méthylène à 2% pour une durée de 12 heures à 37°C, sectionnées et évaluées au niveau de l’interface marginale. Les données ont été analysées statistiquement par le test de l’analyse de variance à deux facteurs suivie par des comparaisons multiples de Tukey HSD.
Résultats : La différence statistique entre les deux types de composites n’a pas été significative (p = 0,682), de même que pour l’effet de l’interaction (p = 0,678). Concernant les techniques de photopolymérisation testées, la différence était significative (p = 0,014). Le test de Tukey HSD a indiqué que la moyenne des scores du groupe A (M = 2.40, SD=1.102) était significativement différente de celle du groupe D (M=1.30, SD=1.393, p=0.010). Pas de différence significative entre le groupe D et le groupe B (M=1.63, SD=1.351, p=0.767), ainsi que le groupe C (M=1.60, SD=1.404, p=0.819). Il y avait une différence entre le groupe A (M=2.40, SD=1.102) et le groupe B (M=1.63, SD=1.351) ainsi que le groupe C (M=1.60, SD=1.404). Cependant, lorsque le test de Tukey HSD a été utilisé cette différence n’était plus statistiquement significative entre le groupe A et les groupes B et C (p=0,122 et p=0.098).
Conclusions : Il y avait une différence statistiquement significative, dans la formation de microhiatus, entre la « polymérisation lente et progressive » et la polymérisation standard, cependant il n’y avait pas de différence statistiquement significative entre les composites nano-chargés et micro-hybrides polymérisés avec les différents modes de polymérisation.
Introduction: To reduce the polymerization shrinkage of the composite resins and after the introduction of the "slow and gradual polymerization" by GORACCI et al. in 1992, many light curing units (LCU) presented the “soft-start polymerization” in addition to the classical high light intensity mode. This study investigated whether this slow and gradual polymerization has the ability of reducing the marginal debonding, by minimizing the internal stress of the composite, knowing that rare are the studies which compared this mode of polymerization.
Material and methods: One hundred twenty Class II cavities were prepared at the mesial and the distal side of human extracted premolars. The teeth were randomly divided into four groups (n=15). In each tooth the mesial Class II cavities were restored with the micro-hybrid resin composite Filtek Z250 (3M ESPE) and the distal Class II cavities were restored with the nano-filled resin composite Filtek Supreme (3M ESPE) The light curing unit used in this study was a conventional quartz tungsten halogen (QTH) curing light, the QHL 75 (Dentsply) modified by adding a regulating electronic device, controlled by a special software that gives 4 different modes of polymerization as follows:
The teeth were stored in water at 37°C for 48 hours, then finished and subjected to thermocycling (3000 cycles between 5°C and 55°C). All the teeth were immersed in 2% methylene blue solution for 12 hours at 37°C, sectioned, and evaluated at the gingival margins. Data were statistically analyzed by two-way analysis of variance followed by Tukey HSD multiple comparisons.
Results: The main effect for the type of composites (p= 0.682), and the interaction effect (p=0.678) did not reach statistical significance. There was a statistically significant main effect for the type of polymerization used (p=0.014). Post-hoc comparisons using the Tukey HSD test indicated that the mean score for the Group A (M=2.40, SD= 1.102) was significantly different from the group D (M= 1.30, SD= 1.393, p= 0.010). The group D did not differ significantly from group B (M= 1.63, SD= 1.351, p=0.767), and group C (M= 1.60, SD= 1.404, p=0.819). There was a difference between the group A (M=2.40, SD= 1.102), group B (M= 1.63, SD= 1.351) and group C (M= 1.60, SD= 1.404). However, when the Tukey HSD was used, no statistically significant differences between Group A, B and C were found (p=0.122 and p =0.098).
Conclusions: There was a statistically significant difference in micro leakage between the “slow and gradual polymerization” and the standard polymerization, however no significant differences were found when using the nano-filled or the micro-hybrid composite materials cured with different polymerization regimens.

Santé parodontale et syndrome de Goujerot-Sjögren. Etude cas-témoins au service de médecine interne

L’objectif de cette étude est de comparer l’état parodontal des patients atteints de SGS comparativement à celui de sujets indemnes du SGS et de rechercher le lien entre la maladie parodontale et le SGS.
Cent trois patients dont 36 avec un SGS primitif et 67 avec un SGS secondaire et 103 témoins ont été sélectionnés pour cette étude. Le niveau d’hygiène a été évalué par l’indice de plaque (plaque index PI) de Silness et Löe et l’inflammation par l’indice gingival (gingival index GI) de Löe et Silness. Les profondeurs de poche et la perte d’attache clinique ont été mesurées à l’aide de la sonde parodontale de Williams.
Les indices de plaque moyens étaient identiques dans les deux groupes (1,27 ± 0,1 vs 1,22 ± 0,1 ; p= 0,67). Comparativement aux témoins, les scores de l’indice d’inflammation étaient significativement plus élevés chez les patients atteints du SGS (IG = 1,1 ± 0,1 vs 0,44 ± 0,2 p < 0,001). Le modèle de régression logistique appliqué à l’ensemble de l’échantillon (cas et témoins) montrait que les patients atteints du SGS avaient plus de risque de développer une maladie parodontale (IG : OR 5,508 IC [1,66-9,60] ; PP : OR= 4,51 IC [1,692-12,024]).
Un programme d’éducation des patients atteints d’un syndrome de SGS paraît essentiel pour limiter les effets délétères d’un terrain bucco-dentaire défectueux. Il permettrait ainsi de convaincre les patients de l’intérêt de la prévention par une surveillance et des soins réguliers.
The aim of this study is to compare the periodontal status of patients with Gougerot-Sjögren syndrome (GSS) to healthy subjects in order to investigate the relation between periodontal disease and GSS.
To achieve this aim, one hundred and three patients, among whom 36 with primary GSS and 67 with secondary GSS, were selected and compared with one hundred and three (103) control subjects. The hygiene level was evaluated with Silness and Löe plaque index (PI) and inflammation level with Löe and Silness gingival index (GI). Probing depth and clinical attachment loss was measured with Williams periodontal probe. Mean plaque indices were identical for both groups (1.27±0.1 versus 1.22±0.1, p= 0.67). Compared with control subjects, the inflammation index scores were significantly higher among SSG patients (IG= 1.1±0.1 versus 0.44±0.2, p < 0,001). The logistical regression model applied to the whole sample showed that patients with GSS had a higher risk of developing periodontal disease (IG : OR 5.508, state 95%CI [1.66-9.60] ; PP : OR= 4.51 95%CI [1.692-12.024]).
A training program for GSS patients seems essential, in order to manage the deleterious effects of defective oral-dental ground. It would allow convincing patients about the interest of prevention through regular surveillance and care.
