Résumé

Contexte : La pérennité de l’ensemble dento-prothétique est un des principes majeurs de conception et de réalisation de la prothèse fixée. Le choix de la dent pilier et le maintien de son intégrité sont deux facteurs indissociables pour atteindre cet objectif.
Objectif : Notre étude s'est proposée d'identifier les différentes complications pulpaires et péri-apicales auxquelles sont sujettes les dents piliers de prothèse fixée en Côte d'Ivoire.
Matériels et méthodes :
Il s’est agi d’une étude transversale descriptive et analytique qui a concerné des piliers de prothèses fixées portées depuis au moins un an. Un examen clinique et des radiographies retro-alvéolaires ont permis d’affiner notre diagnostic et de caractériser les piliers examinés.
Résultats : L’échantillon comprenait 41 dents vivantes et 59 dents dépulpées. Les complications majeures enregistrées sur les piliers étaient les caries secondaires (33%), les sensibilités (27%) et des lésions péri-apicales (19%). Les sensibilités et caries secondaires étaient statistiquement corrélées à un mauvais ajustage cervical (p= 0,0000). La présence d’une lésion péri-apicale étaient statistiquement liée à l’existence de prématurités et/ou d’interférences occlusales (p = 0,036217) et à la mauvaise qualité du traitement endodontique pré-prothétique (p= 0,0000).
Conclusion : De cette étude, il ressort que les complications majeures pouvant compromettre l’intégrité du pilier dentaire et le parodonte de celui-ci sont essentiellement des caries secondaires, des sensibilités et des lésions péri-apicales.

Abstract
Evaluation of pulpal and peri-apical condition of 100 fixed prosthetic pillars. Cross-sectional study conducted in Abidjan, Ivory Coast

Background: The durability of the dento-prosthetic system is one of the main principles of design and realization of the fixed prosthesis. The choice of the abutment tooth and the maintenance of its integrity are two inseparable factors in achieving this objective.
Objective: Our study aimed to identify the different pulpal and periapical complications to which abutment teeth of fixed prostheses are subject in Ivory Coast.
Materials and methods: This was a descriptive and analytical cross-sectional study that involved fixed prosthetic abutments worn for at least one year. A clinical examination and retroalveolar radiographs made it possible to refine our diagnosis and characterize the abutments examined.
Results: The sample included 41 live teeth and 59 pulverized teeth. The major complications recorded on the abutments were secondary caries (33%), sensitivities (27%) and periapical lesions (19%).
Secondary sensitivities and caries were statistically correlated with poor cervical adjustment (p = 0.0000). The presence of a periapical lesion was statistically related to the existence of prematurity and/or occlusal interference (p = 0.036217) and low-quality pre-prosthetic endodontic treatment (p = 0.0000).
Conclusion: From this study, it appears that the major complications that can compromise the integrity of the dental abutment and the periodontal prosthesis are essentially secondary caries, sensitivities and periapical lesions.
