Résumé

La gestion prothétique chez l’édenté complet gériatrique présente une approche spécifique. Outre l’atteinte psychologique (répercussions de l’édentement), le patient est souvent poly-médiqué avec des problèmes digestifs, endocriniens ou autres. Le contexte anatomo-physiologique est par ailleurs souvent défavorable marqué par une résorption accrue qui complique la réhabilitation prothétique et compromet l’équilibre de la Prothèse Amovible Complète (PAC).
En l’absence de toute thérapeutique implantaire, l’enregistrement optimal de la surface d’appui devient capital pour l’intégration prothétique d’un point de vue fonctionnel et psychologique.
L’empreinte piézographique constitue une technique intéressante permettant de déterminer la zone d’équilibre ou espace bio-fonctionnel dans lequel s’inscrira la prothèse. Cette technique correctement conduite permet d’augmenter la rétention physique par l’exploitation des surfaces d’appui tertiaires et d’optimiser l’équilibre occlusal par la détermination du couloir où s’effectue le montage des dents prothétiques avec une situation physiologique du plan d’occlusion.
Les auteurs se proposent à travers ce travail de montrer l’intérêt de cette technique et les étapes-clé dans sa réussite.

Abstract
The piezographic impression in complete edentulous geriatric patient: key to prosthetic stability

Prosthetic management of complete edentulous geriatric person presents a specific approach. In addition to the psychological damage (repercussions of edentulism), the patient is often poly- medicated with digestive, endocrine or other problems. Moreover, the anatomo-physiological context is often unfavourable, marked by increased resorption which complicates prosthetic rehabilitation and compromises the balance of the Complete Removable Prosthesis (CRP).
In the absence of any implant therapy, optimal impression of the support surface becomes crucial for prosthetic integration from a functional and psychological point of view.
The piezographic impression is an interesting technique to determine the equilibrium zone or biofunctional space in which the prosthesis will fit. This technique, when it’s correctly carried out, improves the physical retention and stability of mandibular complete denture. It also optimizes the occlusal balance by guiding the positioning of the prosthetic teeth and a physiological situation of the occlusal plane. Through this work, the authors propose to show the interest of this technique and the key-steps in its success.
