Résumé

Introduction : Le choix de la technique de prise en charge des Communications Bucco-Sinusiennes (CBS) dépend de plusieurs facteurs, notamment la taille de la communication, le temps de l’exposition de l’effraction, le plateau technique mis à disposition. Le corps adipeux buccal appelé corps adipeux de la joue, constitue un moyen simple et efficace dans la fermeture des communications bucco-sinusiennes. Le but de ce travail était d’illustrer l’intérêt du lambeau de boule de Bichat dans la prise en charge des communications bucco-sinusiennes.
Matériels et méthodes : Il s’agissait d’une étude transversale descriptive, qui s’est déroulée de 2020 à 2024. Elle concernait tous les cas de CBS pris en charge par lambeau de boule de Bichat.
Résultats : Onze cas ont été recensés. Le sexe féminin était le plus représenté avec 9 cas (81,8%) et le sex-ratio était de 0,22. La moyenne d’âge était de 38 ans avec un écart-type de 15,78. Les âges extrêmes étaient de 19 ans et 65 ans. Les causes tumorales étaient les plus représentées soit 6 cas (54,5%). Le siège alvéolaire était le plus fréquent avec 7 cas (63,6%). La taille de la communication bucco-sinusienne était supérieure à 30 mm dans 7 cas (63,6%) avec une moyenne de 40,52 mm. La prise en charge chirurgicale consistait en une fermeture par le lambeau de boule de Bichat et l’évolution était favorable dans 9 cas (81,8%) après un recul d’un mois.
Discussion : Les causes des CBS sont iatrogènes le plus souvent. Le lambeau de Bichat est la technique de choix pour les CBS de diamètre supérieur à 5 mm, avec un taux de fermeture supérieur à 98%.
Conclusion : Le lambeau de boule de Bichat est une technique privilégiée dans la prise en charge des CBS en raison de sa simplicité, de son faible taux d’échec.

Abstract
Bichat ball flap in the management of oral-sinus communications at the National Center of odonto-stomatology professor Hamady Traore of Bamako

Introduction: The choice of technique for treating Oro-Sinus Communications (OSC) depends on several factors, including the size of the communication, the time of exposure of the breach, and the technical platform available. The buccal fat body, called the cheek fat body, constitutes a simple and effective means for closing oro-sinus communications. The objective of this work was to illustrate the benefit of the Bichat ball flap in the management of oral-sinus communications.
Materials and methods: This was a descriptive cross-sectional study, which took place from 2020 to 2024 in the stomatology and maxillofacial surgery department of CHU-CNOS Pr HT. It concerned all OSC cases managed by Lambeau de Bichat.
Results: Eleven cases were identified. The female sex was the most represented with 9 cases (81.8%) and the sex-ratio was 0.22. The average age was 38 years with a standard deviation of 15.78. The extreme ages were 19 years and 65 years. Tumor causes were the most represented with 6 cases (54.5%). The alveolar site was the most frequent with 7 cases (63.6%). The size of the oro-sinus communication was greater than 30 mm in 7 cases (63.6%) with an average of 40.52 mm. Surgical management consisted of closure by the Bichat ball flap and the evolution was favorable in 9 cases (81.8%) after a follow-up of one month.
Discussion: The causes of OSC are most often iatrogenic. The Bichat fat ball flap is the technique of choice for OSC with a diameter greater than 5 mm, with a closure rate greater than 98%, but its numerous advantages and excellent results have led to its indication being broadened.
Conclusion: The Bichat flap is a preferred technique in the management of CBS due to its simplicity, its low failure rate and complications of donor site morbidity.
