Résumé

Introduction : La survenue du carcinome épidermoïde de la cavité buccale chez le sujet jeune de moins de 40 ans non-alcoolo-tabagique est de plus en plus décrit par plusieurs auteurs. Nous rapportons un nouveau cas de carcinome épidermoïde de la cavité buccale du sujet jeune qui constitue une entité étiologique à élucider.
Observation : Un patient âgé de 37 ans, sans antécédent de consommation d’alcool ou de tabac, est reçu une première fois en consultation pour une lésion ulcéreuse d’environ 2 cm du bord marginal droit de la langue. Une biopsie incisionnelle a été réalisée et le résultat anatomopathologique a révélé un carcinome épidermoïde. Les sérologies HPV et VIH étaient négatives. La tumeur était classée T2N0M0 et l’indication d’une glossectomie partielle fut posée. Le patient est perdu de vue puis admis à nouveau 4 mois plus tard avec une altération majeure de l’état général. Les explorations cliniques et paracliniques permettaient de classer la tumeur en T4N2bM0. Dans l’attente d’une prise en charge en oncologie, l’évolution a été marquée par le décès du patient.
Conclusion : Les sujets jeunes sans antécédents d’intoxication alcoolo-tabagique présentant des cancers buccaux HPV négatifs constituent une nouvelle population. Notre observation vient attirer l’attention du clinicien sur la possibilité de survenue du carcinome épidermoïde chez le sujet jeune afin de permettre un diagnostic et une prise en charge précoces.

Abstract
Squamous cell carcinoma of the oral cavity in a young non-smoker and non-drinker: presentation of a clinical observation

Introduction: The occurrence of squamous cell carcinoma of the oral cavity in young, non-alcoholic and non-smoking individuals under 40 years of age is increasingly being described by several authors. We report a new case of squamous cell carcinoma of the oral cavity in young individuals, which constitutes an etiological entity that requires further investigation.
Case report: This 37-year-old patient, with no history of alcohol or tobacco use, was initially seen for an ulcerative lesion approximately 2 cm long on the marginal edge of the tongue. An excisional biopsy was performed and the anatomo-pathological result revealed that the patient was squamous cell carcinoma. HPV and HIV serologies were negative. The tumor was classified as T2N0M0, and partial glossectomy was indicated. The patient was lost to follow-up and then readmitted 4 months later due to a major deterioration in his general condition. Clinical and paraclinical investigations allowed the tumor to be classified as T4N2bM0. While awaiting oncology care, the patient’s course was marked by death.
Conclusion: Young subjects without a history of alcohol or tobacco intoxication presenting with HPV-negative oral cancers constitute a new population. Our observation draws the clinician’s attention to the possibility that squamous cell carcinoma occurs in young subjects to enable early diagnosis and management.
