Résumé

Introduction : La microstomie, caractérisée par un rétrécissement de l’orifice buccal, constitue un défi majeur pour la réhabilitation bucco-dentaire, notamment après un traumatisme sévère.
Cas clinique : Nous rapportons le cas d’un patient de 35 ans, victime d’un traumatisme oro-facial balistique lié à des attaques terroristes. À la suite d’une chirurgie réparatrice, il a développé une microstomie sévère, associée à un édentement étendu et à une perte fonctionnelle de la mastication. Une réhabilitation implanto-prothétique a été planifiée après l’ostéointégration réussie des implants maxillaires et mandibulaires. Compte tenu de la microstomie, des empreintes implantaires spécifiques ont été réalisées. Une occlusion en intercuspidie modifiée et une prothèse transitoire ont été mises en place afin de surmonter les difficultés d’adaptation passive des prothèses supra-implantaires. Le suivi radiographique et clinique, après quatre ans, a montré une récupération fonctionnelle complète, un maintien osseux satisfaisant autour des implants et une amélioration notable de la microstomie.
Conclusion : La prise en charge des patients présentant une microstomie sévère requiert une approche prothétique spécialisée, distincte des méthodes classiques, pour assurer une réhabilitation fonctionnelle optimale.

Abstract
Implant-prosthetic rehabilitation in a patient with microstomia secondary to ballistic trauma: case report

Introduction: Microstomia, defined as a reduced oral opening, represents a major challenge in oral rehabilitation, particularly following severe trauma.
Case report: We present the case of a 35-year-old patient who sustained a ballistic oro-facial trauma related to terrorist attacks. Following reconstructive surgery, he developed severe microstomia, extensive tooth loss, and impaired masticatory function. Implant-prosthetic rehabilitation was planned after successful osseointegration of maxillary and mandibular implants. Due to the microstomia, customized implant impressions were performed. A modified intercuspidation occlusion and a provisional prosthesis was placed to overcome difficulties in passive adaptation of the superstructure prostheses. Four-year radiographic and clinical follow-up showed complete functional recovery, stable peri-implant bone support, and significant improvement of microstomia.
Conclusion: Management of patients with severe microstomia requires a specialized prosthetic approach, distinct from conventional methods, to achieve optimal functional rehabilitation.
