Résumé

Introduction : L'otite externe nécrosante progressive ou otite externe maligne est une maladie infectieuse invasive et nécrosante dont le point de départ est une atteinte du conduit auditif externe puis diffusion de l’infection vers les parties molles et l’os adjacent. Le but de ce travail était de rapporter un cas d’otite externe nécrosante progressive chez un enfant non-diabétique afin de discuter des difficultés liées à sa prise en charge dans notre contexte.
Observation : Il s’agissait d’un enfant de 5 ans, de sexe féminin, qui a été admise dans le service d’ORL du CHU mère-enfant "Le Luxembourg" pour otalgie droite dans un contexte de paludisme grave sur terrain de malnutrition protéino-calorique. L’examen clinique a retrouvé une altération de l’état général, une fièvre à 40°C, une otorrhée purulente droite, une large ulcération péri-auriculaire droite et une déviation labiale homolatérale. Le diagnostic d’une otite externe nécrosante progressive droite compliquée d’une paralysie faciale périphérique homolatérale a été évoqué. Une récupération nutritionnelle, une bi-antibiothérapie et des soins locaux ont permis d’obtenir une évolution locale favorable avec amélioration progressive de l’état général.
Conclusion : L’otite externe nécrosante progressive demeure une pathologie redoutable notamment lorsqu’elle existe chez l’enfant, cette dernière est caractérisée par la variabilité du germe en cause et une plus grande fréquence de paralysie faciale. La prise en charge précoce et adéquate de cette pathologie conditionne le succès thérapeutique.

Progressive necrotizing otitis externa complicated with facial paralysis in a child

Introduction: Progressive necrotizing otitis externa or malignant otitis externa is an invasive and necrotizing infectious disease whose starting point is an attack of the external auditory canal and then spread of the infection to the soft tissues and the adjacent bone. The purpose of this work was to report a case of progressive necrotizing otitis externa in a non-diabetic child to discuss the difficulties associated with its management in our context.
Observation: It was a 5-year-old child who was admitted to the ENT department of the mother-child hospital "Luxembourg" for right earaches in a context of severe malaria on a plot of protein-calorie malnutrition. Clinical examination revealed deterioration of general condition fever at 40°C, right
purulent otorrhea, wide right peri-atrial ulceration and ipsilateral labial deviation. The diagnosis of right progressive necrotizing otitis externa complicated by ipsilateral peripheral facial palsy was discussed, Nutritional recovery, bi-antibiotic therapy and local care allowed to obtain a favorable local evolution with progressive improvement of the general state.
Conclusion: The progressive necrotizing otitis externa remains a formidable pathology especially when it exists in the child, the latter is characterized by the variability of the germ in question and a greater frequency of facial paralysis. Early and adequate care of this pathology conditions therapeutic success.
