Objectif : Le but de notre étude est d’analyser les étiologies des péritonites aiguës généralisées opérées dans les hôpitaux de Bangui, ainsi que le niveau de sensibilité aux antibiotiques des souches bactériennes isolées des pus de péritonites.
Patients et méthode : D’avril 2003 à mars 2004, quatre-vingt treize patients présentant de péritonite aiguë généralisée ont été opérés au niveau des trois principaux hôpitaux de Bangui. Pendant l’intervention, l’origine organique a été recherchée. Cinq à 10 ml de liquide ou de pus de péritonite étaient prélevées et analysés au laboratoire à la recherche de germes. Après identification des espèces bactériennes, l’antibiogramme a été réalisé par la méthode de diffusion en milieu gélose Muller Hinton.
Résultats : Les péritonites aiguës généralisées ont représenté 12,5 % des syndromes douloureux abdomino-pelvien non-traumatiques opérés (93/746). L’échantillon était constitué de 65,6 % d’hommes et 34,4 % de femmes (sex-ratio = 1,9). L’âge moyen des patients était de 25,8 ans (extrêmes : 4-66 ans). La tranche d’âge de 15 à 29 ans était le plus souvent atteinte (37,6 %). Les appendicites aiguës ont représenté la première cause des péritonites (40,8 %). Elles sont suivies des causes gynécologiques (16,1%) puis des causes intestinales (15,0 %). Aucune cause organique n’a été retrouvée dans 27,9 % des cas. Les prélèvements analysés étaient positifs dans 78,5 % des cas. Ils étaient polymicrobiens et on notait une prédominance d’Escherichia coli (57,5 %) et le streptocoque (30,1 %). Ces bactéries étaient constamment sensibles à la ciprofloxacine (100 %) et à la gentamicine (75 à 95,5 %).
Conclusion : Les péritonites aiguës généralisées sont souvent secondaires à une cause digestive, l’appendicite occupant une place de choix. L’antibiothérapie probabiliste de première intention de cette affection devra utiliser un dérivé des quinolones en monothérapie ou en association avec un aminoside.
Aim : The purpose of the study was to analyze the aetiologies of the generalized acute peritonitis operated in Bangui, and to identify the germs, which isolated from the pus from peritonitis then determining their sensibility to the currently used antibiotics. It was a prospective study performed from April 2003 to March 2004.
Method : The sample was set up by ninety three patients presenting generalized acute peritonitis operated in one of the three principal hospitals of Bangui. During the intervention, the organic origin was required. Five to 10 ml of liquid or pus of peritonitis were taken and analyzed at the laboratory to search germs. All specimens have been submitted to a macroscopic, microscopic examination in fresh state, examination after staining (May-Grumwald-giemsa, Gram and blue staining methylen). The Ziehl-Nielsen stains were performed when the tow previous stains are negative. These specimens were also sowing on three classical culture aspects. After identification of the bacterial species, the antibiogram was carried out by the method of diffusion in Muller Hinton Agar medium. The tested antibiotics were those available in generic form.
Results : The generalized acute peritonitis accounted for 12,5% of the painful syndromes abdominal-pelvic no traumatic operated (93/746). The sample was constituted by 65,6% of men and 34,4% of women (sex-ratio = 1,9). The average age of the patients was 25,8 years (extremes : 4 - 66 years). The age bracket from 15 to 29 years was generally the more reached one (37,6%). Acute appendicitis represented the first cause of peritonitis (40, 8%), followed by gynaecological causes (16,1%) then intestinal causes (15,0%). No organic cause was found in 27,9% of the cases. The analyzed samples were positive in 78,5% of the cases. They were poly microbial and one noted a prevalence of Escherichia coli (57, 5%) and streptocoque (30,1%). These bacteria were constantly sensitive to Ciprofloxacin (100%) and gentamicin (75 to 95,5%).
Conclusion : The generalized acute peritonitis is often secondary to a digestive cause, appendicitis taking a chosen place. The first-line probabilistic antibiotherapy of this affection will have to use a derivative of quinolon in monotherapy or be associated with an aminosid.
