Le but de ce travail est d'analyser les aspects épidémiologiques thérapeutiques et évolutifs du choléra à l'Hôpital d'Enfants de Dapaong au Togo.
Nous avons réalisé une étude rétrospective portant sur 115 malades âgés de 3 mois à 10 ans hospitalisés pour choléra en période d'épidémie du 1er juillet au 31 octobre 1998. Les tranches d'âge de 3 à 4 ans et de 7 à 8 ans ont été les plus touchées avec respectivement 23,47 % et 21,73 % des cas. Tous les malades ont présenté à leur admission une déshydratation sévère consécutive à une diarrhée aiguë aqueuse grave associée ou non à des vomissements.
Du point de vue thérapeutique, les malades ont été réhydratés soit par voie veineuse avec du Ringer lactate dans 63,48 % soit par voie orale avec la solution de réhydratation orale (S.R.O.) de l'UNICEF dans 36,52 % des cas. Les antibiotiques utilisés ont été la tétracycline chez 37,40 % des malades et le cotrimoxazole chez 62,60 % des enfants de moins de 7 ans. L'évolution clinique a été favorable dans 85,22 % des cas ; le taux de létalité est de 14,78 %.
La prévention de cette affection repose essentiellement sur des mesures hygiéno-diététiques.
The aim of this work is to bring out the epidemic, the therapeutic and the developing aspects of cholera at the children's hospital in Dapaong (Togo).
A retrospective study was done on 115 patients aged from 3 months to 10 years who were suffering from cholera and were admitted to the hospital during the epidemic period from 1st July to 31st October 1998. The most affected children were those aged between 3 to 4 years and 7 to 8 years with 23,47 % and 21,73 % cases respectively. All those children at their admission presented a severe dehydration due to chronic watery diarrhoea going with or without vomiting.
The patients were cured by rehydratation either with Ringer lactate through the veins in 63,48 %, or with the oral rehydratation solution (S.R.O.) of the UNICEF in 36,52 % of the cases. The antibiotics used are tetracyclin for 37,40 % of the patients and cotrimoxazole in 62,60 % cases for children under 7 years. The clinical evolution was favourable in 85,22 % of the cases ; the rate of lethality is 14,78 %. The prevention of such an affection is to be based essentially on hygienic and dietary measures.
