Introduction : Le sarcome de Kaposi est une pathologie dont la fréquence est augmentée depuis l’avènement du VIH/SIDA. Au cours de son évolution, la maladie de Kaposi se complique rarement de pancytopénie. Nous rapportons deux cas de thrombopénie sévère d’évolution fatale probablement dus à un syndrome d’activation macrophagique au cours de la maladie de Kaposi.
Observations : Une femme âgée de 43 ans était hospitalisée pour une maladie de Kaposi-SIDA le 15 mai 2006. Un hémogramme objectivait une anémie sévère avec l’hémoglobine à 4,6 g/dl et des plaquettes à 36000/ul. Elle a bénéficié du 16 mai au 10 juillet 2006 de 5 litres de culot globulaire, de 4 séances de perfusion de bléomycine et d’une corticothérapie. Malgré ce traitement, l’hémoglobine et les plaquettes ont continué à chuter. La patiente décédait au bout de 2 mois dans un tableau de détresse respiratoire.
Un homme de 25 ans atteint de la maladie de Kaposi Africain depuis 1989 a été hospitalisé en 2002 pour complication hémorragique et dissémination de la maladie. Le patient était porteur d’une hémoglobinopathie AC et l’hémogramme objectivait une anémie sévère avec l’hémoglobine à 5,9 g/dl et une thrombopénie sévère à 18000/ul. Le patient a bénéficié de 8 litres de transfusion de culot globulaire, d’une corticothérapie et d’une chimiothérapie à base de Vinblastine puis de bléomycine. Toutes ces thérapeutiques ont été inefficaces et le patient décédait dans un tableau de thrombopénie sévère (2000/ul).
Discussion : La maladie de Kaposi se complique rarement de pancytopénie par séquestration des éléments figurés du sang dans les structures et vaisseaux anormaux, mais la survenue d’une pancytopénie sévère évoque un syndrome d’activation macrophagique. Dans nos deux observations, la responsabilité de l’activation macrophagique est probable du fait de la persistance et de l’aggravation de cette pancytopénie malgré une prise en charge adéquate. Un myélogramme serait nécessaire pour confirmer cette hypothèse d’activation macrophagique mais n’a pu être réalisé à cause du risque de saignement.
Introduction: Kaposi’s sarcoma frequency is increased since the advent of HIV/AIDS. During its evolution, pancytopenia didn’t often complicate Kaposi’sarcoma disease. We report two cases of seve-re thrombopenia with fatal evolution probably caused by macrophagic activation syndrome during the Kaposi’s sarcoma.
Observations: A 43 year-old woman was hospitalized for epidemic Kaposi’s sarcoma on 15th May, 2006. Hemogram objectified severe anaemia with 4,6 g/dl of hemoglobin and 36000 platelets/ul. She received 5 liters of globular sediment transfusion from 10th May to 16th July, 2006, 4 sessions of Bleomycin perfusion and corticotherapy. In spite of this treatment, hemoglobin and platelets continued to fall. The patient died after 2 months in breathing difficulties condition.
A 25 year-old man suffering from endemic Kaposi’s sarcoma since 1989 was hospitalized in 2002 for haemorrhagic complication and dissemination of the disease. The patient was carrying AC haemoglobinopathy and hemogram objectified severe anaemia with 5,9 g/dl of hemoglobin and thrombopenia (18000/ul). The patient received 8 liters of globular sediment transfusion, corticotherapy and Vinblastin then Bleomycin chemotherapy. All these therapeutic were ineffective and the patient died in a severe thrombopenia (2000/ul) condition.
Discussion: Kaposi’s sarcoma didn’t become complicated by pancytopenia by sequestration of blood constituents in the structures and vessels abnormal, but a severe pancytopenia evokes a macrophagic activation syndrome. In our observations, the responsibility of macrophagic activation is probable because of persistence and aggravation of this pancytopenia in spite of an adequate treatment. A myelogram might be necessary to confirm this hypothesis of macrophagic activation but could not be achieved because of the risk of bleeding.
