Objectif : Actualiser les aspects histopathologiques et bactériologiques des ulcères gastroduodénaux à Dakar (Sénégal) afin d’adapter l’antibiothérapie d’éradication d’Helicobacter pylori (H. pylori).
Patients et méthode : Il s’agissait d’une étude transversale incluant les patients ulcéreux diagnostiqués dans le centre d’endoscopie digestive du CHU Aristide Le Dantec de Dakar (Sénégal) sur une période de 6 mois. Les patients avec ulcère hémorragique ou cancérisé et ceux ayant pris dans le mois précédent un traitement anti sécrétoire gastrique, antibiotique ou anti inflammatoire ont été exclus. Huit biopsies (4 antrales et 4 fundiques) ont été pratiquées pour histologie et culture de H. pylori avec antibiogramme. Etaient analysés l’âge des patients, le sexe, le siège des ulcères, les lésions histologiques, les résultats des cultures et les antibiogrammes.
Résultats : 58 patients avec ulcère duodénal (âge moyen de 41 ans et sex-ratio de 1,4) et 44 avec ulcère gastrique (âge moyen de 48,4 ans et sex-ratio de 1,3) ont été étudiés. Une gastrite chronique existait chez tous les patients et elle était associée à une atrophie dans 41,2% des cas et une métaplasie dans 24,5% des cas. L’histologie et la culture ont montré la présence de H. pylori dans 84,3% des cas. Les antibiogrammes ont révélé une sensibilité de 100% à l’amoxicilline, 96,2% à la clarithromycine, 84,6% à la ciprofloxacine et 19,2% au métronidazole.
Conclusion : L’ulcère gastroduodénal est jusqu’à présent fortement lié à l’infection à H. pylori à Dakar. Les schémas d’éradication de H. pylori à Dakar devraient tenir en compte le taux élevé de résistance in vitro au métronidazole.
Objective: To update histopathological and bacteriological aspects of peptic ulcers in Dakar (Senegal) in order to adapt the antibiotic for eradication of Helicobacter pylori (H. pylori).
Patients and method: It was a cross-sectional study including patients with peptic ulcer diagnosed in the digestive endoscopic center of the Teaching Hospital Aristide Le Dantec of Dakar (Senegal) over a 6-month period. Patients with haemorrhagic or malignant ulcer and those who took protons pump inhibitors, antibiotic or anti inflammatory treatment in the previous month were excluded. Eight biopsies (4 antral and 4 fundal) were performed for histology and culture of H. pylori with antibiogram. Patient’s age and sex, site of ulcers, histological lesions, and results of cultures and antibiotic susceptibility testing were analysed.
Results: 58 patients with duodenal ulcer (mean age 41 years and sex-ratio 1.4) and 44 with gastric ulcer (mean age 48.4 years and sex-ratio 1.3) have been studied. Chronic gastritis was found in all patients and it was associated with atrophy in 41.2% of the cases and metaplasia in 24.5 % of the cases. Histology and culture showed the presence of H. pylori in 84.3 % of the cases. Antibiotic susceptibility testing revealed sensibility of H. pylori in 100% of the cases to amoxicilline, 96.2% to clarithromycine, 84.6% to ciprofloxacine and 19.2% to metronidazole.
Conclusion: Peptic ulcer is still strongly linked to H. pylori infection in Dakar. Schemes for the eradication of H. pylori in Dakar should take into account the high rate of in vitro resistance to metronidazole.
