Les infections postopératoires dans l’ostéosynthèse des fractures de jambe et du pilon tibial demeurent très élevées.
Cette étude vise à déterminer l’incidence des infections postopératoires des fractures de la jambe et du pilon tibial traitées par ostéosynthèse et proposer de moyens pour diminuer les infections post-opératoires. Ce travail rétrospectif a été réalisé sur 76 fractures de la jambe et du pilon tibial traitées par ostéosynthèse avec un recul moyen de deux ans.
Le taux d’infection superficielle des sites des broches de fixateur externe a été de 54,24%. Le taux d’infection superficielle et profonde de la plaie opératoire a été de 31,58% et 7,89%. De même, le taux d’ostéite a été important, 7,89%. La fréquence élevée des infections postopératoires était multifactorielle : conditions opératoires difficiles en raison de :
L’amélioration de la politique de l’accès rapide aux soins traumatologiques d’urgence, de l’hygiène hospitalière et le respect de l’asepsie chirurgicale devraient donc réduire sensiblement ce taux d’infection encore très élevé.
The postoperative infections in the osteosynthesis of the fractures of leg and the tibial pilon remain very high.
This study will determine the incidence of postoperative infections of the fractures of the leg and the tibial pilon treated by osteosynthesis and propose means to decrease the postoperative infections. This retrospective work was completed on 76 fractures of the leg and the tibial pilon treated by osteosynthesis with hindsight of 2 years.
Rate of superficial infection of the sites of the pins of external fixator was of 54.24%. The rate of superficial and deep infection of the operational wound was significantly high : 31.58% and 7.89%. In the same way, the rate of deep bony infection was important, 7.89%. The improvement of the policy of rapid access to traumatologic care emergency, hospital hygiene and the respect of the surgical asepsis should thus appreciably reduce this rate of infection still very high.
