Objectif : Le goitre à développement thoracique n’est pas rare. L’objectif de ce travail a été de rapporter les caractéristiques cliniques, radiologiques, histologiques et thérapeutiques de deux cas pris en charge à Cotonou.
Matériel et méthode : Les auteurs ont rapporté dans une étude rétrospective deux cas de goitre à développement thoracique pris en charge au Centre National Hospitalier Universitaire (CNHU) de Cotonou en 2007.
Résultats : Les malades âgés de 34 ans et 54 ans étaient de sexe féminin. La tuméfaction cervicale basse a été retrouvée dans les deux cas. Les signes accompagnateurs ont été l’asthénie, les palpitations et les précordialgies chez l’une des patientes. L’autre a présenté une dyspnée d’effort et de décubitus avec voix bitonale puis une dyspnée laryngée. Le délai de consultation des patientes a été respectivement de 5 ans et 10 ans. Le diagnostic a été basé sur l’examen clinique, la radiographie et la biologie. La sanction thérapeutique a été une thyroïdectomie totale par voie cervicale pure associée à une opothérapie. Une trachéotomie de sécurité a été réalisée en fin d’intervention dans un cas. L’examen anatomopathologique des pièces opératoires a révélé un goitre nodulaire colloïde et kystique et un goitre colloïde multi-nodulaire remanié par des dilatations kystiques. Les suites opératoires ont été simples.
Conclusion : L’imagerie médicale a une place de choix dans le diagnostic du goitre à développement thoracique dont les manifestations cliniques ont été des signes de compression. La chirurgie par abord cervical avec des manoeuvres particulières ont été suffisantes pour l’extraire.
Objective: Goiter with an endothoracic development isn’t rare. The aim of this study was to report clinical, radiological, histopathological and therapeutic characteristics of two cases managed at Cotonou.
Material and method: It was a retrospective study of two cases managed in 2007 at the National teaching hospital of Cotonou.
Results: Patients were women of 34 and 54 years old. Palpable cervical mass was found in two cases. The symptoms were fatigue, palpitations and pain in the chest with one. The second had presented a dyspnea after exertion and at rest with voice fluster following by laryngeal dyspnea. The time-limit of consultation was respectively 5 and 10 years. Diagnosis was based on clinical exam, medical imagery and biology. The treatment was total thyroidectomy by cervical approach associated with opotherapy. In one case, tracheotomy was required at the end of thyroidectomy. Histopa-thological results were nodular colloid goiter and any complications were noticed during the follow-up.
Conclusion: Medical imagery is the key examination of the assessment of the goiter with endo-thoracic development. Its clinical signs were thoracic compression signs. Thyroidectomy was performed via cervical incision and the complete dissection of the cervical portion facilitated the ascension of his endothoracic portion.
